单一切口胸腔镜手术治疗22例肺结核瘤

摘　　要：目的探讨单一切口胸腔镜手术治疗肺结核瘤的价值。方法全身麻醉下,单一部位切口选择在患侧腋中线至腋前线之间第5或6肋间,长3~3.5 cm,采用10 mm 30°胸腔镜,应用腔镜器械楔形切除病灶。结果手术均顺利完成,手术时间（57.5±15.2）min,术中出血量（31.8±8.9）ml。无一并发支气管胸膜漏、脓胸、结核播散及呼吸功能不全。胸管引流量（178.6±27.8）ml,引流时间（3.4±1.1）d,术后住院时间（8.0±0.6）d。22例随访8~30个月,平均16.5月,恢复工作或正常生活,未见复发。结论单一切口胸腔镜下楔形切除肺结核瘤疗效确切。Objective To investigate clinical value of single-incision thoracoscopic surgery for the treatment of pulmonary tuberculoma. Methods Under the general anesthesia,a single incision 3- 3. 5 cm in length was made between the axillary midline and the anterior axillary line at the fifth or sisth intercostal space. By using a 10 mm 30° thoracoscope,pulmonary wedge resection was carried out. Results All the operations were successfully completed,with no complications. The operation time was（ 57. 5 ± 15. 2）min and the intraoperative bleeding volume was（ 31. 8 ± 8. 9） ml. No bronchopleural fistula,empyema,disseminated tuberculosis or respiratory insufficiency happened. The chest tube drainage volume was（ 178. 6 ± 27. 8） ml,the drainage time was（ 3. 4 ± 1. 1） d,and the postoperative hospital stay was（ 8. 0 ± 0. 6） d. Follow-ups for 8- 30 months（ mean,16. 5 months） in 22 cases showed return to work or normal life,without recurrence. Conclusion Single-incision thoracoscopic surgery in the treatment of pulmonary tuberculoma has good efficacy.
