外科手术与急诊内镜手术治疗AOSC患者的随机双盲对照研究

摘　　要：目的:随机双盲对照研究外科手术与急诊内镜手术治疗急性梗阻性化脓性胆管炎(AOSC)患者的效果。方法:随机选取2012年8月至2015年8月我院收治的66例AOSC患者,依据随机数字表法将患者分为两组,即研究组(n=33)和对照组(n=33)。给予对照组患者外科手术治疗,给予研究组患者急诊内镜手术治疗,然后对两组患者的黄疸减轻时间、胃肠功能恢复时间、腹痛缓解时间、引流管拔除时间及住院时间、治愈、中转及死亡情况、术后并发症发生情况进行统计分析。结果:研究组患者的黄疸减轻时间、胃肠功能恢复时间、腹痛缓解时间、引流管拔除时间及住院时间均显著短于对照组(P<0.05),治愈率97.0%(32/33)显著高于对照组84.8%(28/33)(P<0.05),死亡率0显著低于对照组15.2%(5/33)(P<0.05),术后并发症发生率3.0%(1/33)显著低于对照组12.1%(4/33)(P<0.05)。结论:急诊内镜手术治疗AOSC患者的效果较外科手术好。Objective： To investigate the effect of surgery and emergency endoscopic surgery in the treatment of AOSC by randomized double-blind controlled study. Method： 66 cases with AOSC in our hospital from August 2012 to August 2015 were selected as the objects in the study. According to the random number table these patients were divided into two group,study group（ n = 33） and control group（ n = 33）. The control group were given surgical treatment,while the study group were given emergency endoscopic surgery.And then,the reduce time of jaundice,the recovery time,pain relief time of abdominal pain,the removal drainage tube time and the time of hospital stay,the conditions of healing,transit and mortality,postoperative complications were analyzed in the two groups and compared. Result： The reduce the time of jaundice,the recovery time of gastrointestinal function,the relief time of abdominal pain,the removal time of drainage tube and the time of hospital stay in study group were significantly shorter than in control group（ P〈0. 05）. The cure rate in study group was 97. 0%（ 32 /33）,much higher than 84. 8%（ 28 /33） in control group（ P〈0.05）. The mortality rate was 0,significantly lower than 15. 2%（ 5 /33） in control group（ P〈0. 05）. The postoperative complication rate was 3. 0%（ 1 /33）,lower than 12. 1%（ 4 /33） in control group（ P〈0. 05）.Conclusion： The effect of emergency endoscopic surgery was better than surgery in the treatment of AOSC.
