无创通气对急性左心衰竭患者hs-CRP及NT-proBNP的影响

摘　　要：目的 探讨无创通气对急性左心衰竭患者的治疗作用及对患者血浆高敏C反应蛋白（hs-CRP）、N末端B型脑钠肽（NT-proBNP）水平的影响。方法 选择2013年1月~2015年8月在我院心内科住院治疗的急性左心衰竭患者77例,采用随机数字表法将其分为观察组38例及对照组39例。两组均给予综合药物治疗,对照组给予文丘里面罩吸氧,观察组给予无创通气。分析比较两组疗效及治疗前后hs-CRP、NT-proBNP水平变化。结果 两组治疗后均能有效改善患者心功能,降低hs-CRP、NT-proBNP水平。观察组总有效率为92.10%,对照组总有效率为76.92%,两组总有效率比较差异有统计学意义（P〈0.01）。治疗前观察组血浆hs-CRP、NT-proBNP水平与对照组比较,差异无统计学意义（P〉0.05）;治疗后24、72 h,观察组血浆hs-CRP、NT-proBNP水平明显低于对照组（P〈0.05）。结论 无创通气能提高急性左心衰竭治疗效果,且能显著降低患者血浆hs-CRP、NT-proBNP水平。Objective To investigate the therapeutic effect of non-invasive ventilation on patients with acute left heart failure and the levels of high sensitive C reactive protein （hs-CRP）,N terminal B type natriuretic peptide（NT-proBNP） in Plasma. Methods 77 patients with acute left heart failure treated in the cardiological department of our hospital from January 2013 to August 2015 were selected.Patients were randomly divided into observation group including 38 cases and control group including 39 cases.The two groups were given comprehensive drug treatment,the control group was given venturi mask oxygen inhalation,the observation group was given non-invasive ventilation.Changes of hs-CRP,NT- proBNP levels before and after treatment were compared and carried trough correlation analysis. Results The effective rate of the observation group was 92.10%,the effective rate of the comparison group was 76.92%,there was significant difference between the two groups （P〈0.01）.There was no significant difference for hs-CRP and NT-proBNP between two groups before treatment,but these index significantly decreased on 24,72 h after the treatmen（P〈O.05）. Conclusion Noninvasive ventilation can improve the treatment effect of acute left heart failure,and can significantly reduce the hs- CRP,NT-proBNP level in plasma.
