老年带状疱疹后遗神经痛的危险因素研究

摘　　要：目的研究危险因素与60岁及以上老年人带状疱疹后遗神经痛（postherpetic neuralgia，PHN）发生相关的危险因素。方法对340例老年急性带状疱疹患者进行了问卷调查。问卷包含患者的基本信息、症状体征、初始VAS评分、初始IDPain评分、疼痛持续时间等。结果18．8％的患者有神经后遗痛，年龄大于75岁的PHN发生率为31．7％。PHN组平均年龄（77．5±9．3）岁，高于对照组（无PHN）（65．7±16．4）岁，两组差异有统计学意义（P〈0．05）。PHN组平均初始VAS评分（7．73±1．82）分高于对照组（5．08±2．36）分，两组差异有统计学意义（P〈0．05oPHN组平均初始IDPain评分（4．87±0．42）分高于对照组（2．68±1．12）分，两组差异有统计学：意义（P〈0．05）。Logistic回归分析结果显示年龄大于75岁，PHN发生率是60～65年龄组8倍左右（OR=7．978，P〈0．05）；初始VAS评分每增加1分，PHN发生率大约增加0．4倍（OR=1．389，P〈0．05）；初始IDPain评分每增加1分，PHN的发生率大约增加32倍（OR=32．953，P〈0．05）。年龄大于75岁（OR=7．978，P〈0．05）、初始VAS评分（OR=1．389，P〈0．05）、初始IDPain评分（OR=32．593，P〈0．05）与PHN相关。结论年龄大于75岁，初始VAS评分越高或初始IDPain评分越高，患PHN风险越大。Objective To study risk factors relating to postherpetic neuralgia （PHN） in the elderly patients. Methods 340 cases of elderly patients with acute herpes zoster conducted a questionnaire. Questionnaire contained basic information about the patient, signs and symptoms, pain VAS score, ID Pain score, duration of pain and so on. Results 18.8% of the patients had PHN, the incidence of PHN reached 31.7% in patients older than 75 years. Average age of PHN group （77.5±9.3） was statistically significantly higher than that of the control group （65.7 ± 16.4） （P〈 0.05）. The mean initial VAS score of PHN group （7.73± 1.82） was statistically significantly higher than that of the control group （5.08±2.36） （P 〈 0.05）. The mean initial IDPa in score of PHN group （4.87±0.42） was statistically significantly higher than that of the control group （2.68 5±1.12） （P〈 0.05）. Logistic regression analysis showed that the incidence of having PHN after age of 75 years was eight times than the group of 60-65 years （OR=7.978, P〈0.05）; with each one point increase of the initial VAS score, the incidence of PHN increased by about 0.4 times （OR = 1.389, P 〈 0.05）; with each one point increase of the initial ID Pain score, the incidence of PHN increased by about 32 times （OR = 32.953, P 〈 0.05）. Conclusions The age older than 75 years, the higher initial VAS score or initial IDPa in score, the higher incidence of PHN.
