环甲膜快速穿刺通气在急性喉梗阻抢救中的应用价值

摘　　要：目的探讨环甲膜快速穿刺通气在急性喉梗阻抢救中的临床应用,以期缩短抢救时间,减少并发症,降低病死率。方法利用特制的环甲膜穿刺通气装置,在临床上对68例各种原因引起的急性喉梗阻患者,进行环甲膜穿刺置管,并利用专用设备辅助通气。结果临床上全部68例患者均在紧急情况下,顺利完成环甲膜穿刺切开置管术,术中均一次性置管成功;术中及术后随访6个月~2年均无出血、皮下气肿、食管气管瘘、声音嘶哑等并发症发生。结论利用特制的环甲膜穿刺通气装置抢救急性喉梗阻的患者安全有效,具有操作简便快捷,定位准确、切开深度适宜、不易损伤气管后壁和食管、无并发症等优点。Objective To investigate the development and application of a simple one-time cricothyroid membrane fast trocar in acute laryngostasis. Methods 68 patients with acute laryngostasis performed cricothyroid laryngotomy with the device to get effective ventilation. Results All the patients with acute larygostasis performed cricothyroid laryngotomy successfully. The cricothyroid membrane puncture ventilation device can be divided into two parts, portable and effective. All the patients performed a postoperative follow-up of 6 months and 2 year,all the patients without serious complications of emphysema, hemorrhage, tracheo-esophageal fistula and hoarseness. Conclusion The research and production of this instrument can rescue the patients with laryngostasis successfully, safely and effectively. It has the advantages of convenient operation and no complications.
