摘  要:【目的】研究在人体外受精过程中提早剥除颗粒细胞对受精和发育的影响。【方法】对本中心行短时受精(5 h)的53对夫妇纳入研究,当其精卵孵育5 h之后,将所用卵子平均分为2组(5 h组和19 h组),分别于受精5 h和19 h时剥除颗粒细胞。观察两组中同一批卵子受精情况并用荧光原位杂交(FISH)技术分析多原核胚胎中原核(PN)来源。【结果】5 h组和19 h组两组之间的正常受精率(两原核率)、单原核率和未成熟卵率相似,差异均无统计学意义,然而多原核率在5 h组明显高于19 h组﹝(10.9±12.4)%vs.(4.5±7.6)%,P=0.001﹞。同时在5 h组中正常受精卵子原核的直径在受精后19 h时明显大于19 h组的正常受精卵子原核的直径[(24.27±2.20)μm vs.(20.90±3.14)μm,P=0.0001﹞。培养至第3天时两组的可利用胚胎率和优质胚胎率相似,差异无统计学意义。FISH的结果显示,两组多原核胚胎中原核的Y染色体/X染色体的比例为0.533~0.547,理论上更接近于多精受精。【结论】提早剥除颗粒细胞不影响人胚胎的体外发育,但是可能会影响原核的发育并导致体外受精过程中多精受精比例的增加。【Objective】 To investigate the effects of cumulus cells removal after 5 h co-incubation of gametes on the fertilization in human IVF. 【Methods】 A total of 53 cycles undergoing 5 h co-incubation of gametes were included in this study. Sibling oocytes of each patient were assigned to either the 5 h group or the 19 h group. Cumulus cells were mechanically removed either 5h postinsemination or 19 h post-insemination. The status of fertilization,in vitro development and the source of pronuclei in abnormal zygotes were analyzed between sibiling oocytes. 【Results】 No difference was observed between 5h group and 19 h group regarding the rates of normal fertilization(2 pronuclei, 2PN), one pronucleus(1PN), immaturaloocytes. However, significant increase of polypronuclear(3PN) was observed in oocytes of 5h groups compared with that of 19 h group(10.9 ± 12.4% vs 4.5) w6%,P = 0.001). The diameter of pronuclei in fertilized oocytes in the 5 h group was significantly greater than that in the 19 h group(24.27 ± 2.20 μm vs 20.90 ±3.14μm, P=0.0001). The available embryos and optimal embryos rate were not different between 5 h and 19 h groups in day 3. The ratio of sex chromosome in 3PN zygotes was 0.533-0.547. 【Conclusion】 Embryo developmental potential is not influenced by cumulus cells removal after 5 h co-incubation of gametes. However, it would possibly affect the development of pronuclei and cause polyspermy.
