河南开封农村失能老年人健康行为及影响因素研究

摘　　要：目的了解河南开封农村失能老年人健康行为状况及影响因素,为制定健康行为干预措施提供参考依据。方法采用随机整群抽样方法,运用一般情况调查表、一般自我效能量表(GSES)、抑郁自评量表(SDS)和健康促进生活方式评定量表II(HPLP II),对开封市446名农村失能老年人进行入户调查。结果农村失能老年人健康行为总分为(109.73±16.80)分,各维度平均分由高到低依次为人际关系(2.35±0.43)、压力应对(2.34±0.37)、营养(2.29±0.30)、自我实现(2.08±0.49)、健康责任(1.83±0.39)、运动(1.76±0.47)分。二分类Logistic回归分析显示,自我效能(OR=0.796)、文化程度(OR=0.335)是农村失能老年人健康行为的保护因素,失能程度(OR=3.051)、抑郁(OR=1.084)是健康行为的主要危险因素(P<0.05)。结论农村失能老年人的健康行为水平有待提高,在制定干预措施时,要充分考虑影响农村失能老年人健康行为的主要因素,改善老年人的不良行为,促进其身心健康。Objective The study aimed to assess the status of health behaviors and the influencing factors among disabled elderly in rural areas in Kaifeng City of Henan Province,so as to provide a reference for developing intervening measures to improve the health behaviors of disabled elderly. Methods A total of 446 disabled elderly in rural Kaifeng were selected by random cluster sampling,and was surveyed by using a general information questionnaire,General Self- efficacy Scale（ GSES）,Self- rating Depression Scale（ SDS）,and Health Promoting Lifestyle Profile II（ HPLP II）. Results The total score on health behavior of the disabled elderly was 109. 73 ± 16. 80. The average score on each of the dimensions,ranked from high to low,was 2. 35 ± 0. 43 for interpersonal relationship,2. 34 ± 0. 37 for pressure handling,2. 29 ± 0. 30 for nutrition,2. 08 ± 0. 49 for self- realization,1. 83 ± 0. 39 for health responsibility,and 1. 76 ± 0. 47 for sports. Dichotomous logistic regression analysis showed that self- efficacy（ OR = 0. 796） and literacy（ OR = 0. 335） were the protective factors for health behaviors of the disabled elderly; while the level of disability（ OR = 3. 051） and depression（ OR = 1. 084） were the main risk factors for their health behaviors（ P〈0. 05）. Conclusion Health behaviors of the disabled elderly in rural Kaifeng need to be improved. Intervening measures should be designed according to the main influencing factors for their health behaviors,so as to correct unhealthy behaviors and enhance mental health of the disabled elderly.
