中药注射剂联合化疗治疗食管癌有效性的网状Meta分析

摘　　要：目的间接比较不同中药注射剂联合化疗治疗食管癌的疗效,为临床实践与研究提供参考。方法计算机检索PubMed、The Cochrane Library、EMbase、中国生物医学文献数据库、万方数据库、维普数据库和中国知网数据库,同时补充其他检索获取所有关于中药注射剂联合化疗治疗食管癌的随机对照试验,检索时间截止到2014年11月。南2名研究者严格按照纳入与排除标准独立筛选文献、提取数据和质量评价,采用WinBUGS软件进行网状Meta分析。结果研究共纳入9种中药注射剂,26个随机对照试验,包含1739例患者。网状Meta分析结果显示:以化疗为共同干预措施,复方苦参注射液+化疗和华蟾素注射液+化疗分刖与单纯化疗比较,差异有统计学意义,可提高治疗的有效率,其他注射剂+化疗与单纯化疗比较差异无统汁学意义;康莱特注射液+化疗与单纯化疗比较,可改善患者生活质量,差异有统汁学意义,其他均无统计学意义。结论有效率方面推荐消癌平注射剂或康莱特注射剂,而鸦胆子油乳注射剂或消癌平注射剂在提高食管癌患者生活质量方面效果好。Objective To indirect compare the curative effect of different traditional Chinese medicine（TCM）combined chemotherapy in the treatment of esophageal cancer,and provide references for clinical practice and research.Methods PubMed,The Cochrane Library,EMbase and CBM,WanFang,VIP and CNKI databases were searched from inception to November 2014,adding other retrieves all about the randomized controlled trial（RCT） of TCM injection combined chemotherapy versus chemotherapy alone in the treatment of esophageal.Literature selection,data extraction and quality assessment were extracted by two independent reviewers according to the inclusion and exclusion criteria.Network Meta-analysis was performed using WinBUGS software.Results A total of 9 kinds of TCM injections,26 RCT containing 1 739 patients were included in our review.Meta-analysis results show that when comparing with chemotherapy alone as common intervention,treatment effectiveness of compound Kushen injection or huachansu injection plus chemotherapy were significantly enhanced.There were no statistically significant differences in treatment effectiveness between other injection plus chemotherapy and chemotherapy alone.The only difference in the quality of life was found statistically significant between Kanglaite injection plus chemotherapy and chemotherapy.Conclusion Xiaoaiping injection and Kanglaite injection were better than others in the improvement of effectiveness,and Yadanziyouru injection or Xiaoaiping injection is much better for quality of life.
