老年肾移植受者术后服药依从性影响因素的调查研究

摘　　要：目的 :调查老年肾移植受者术后服药依从性的状况,并分析其影响因素。方法 :采用便利抽样法,选取在北京市某三级甲等医院肾移植随访门诊随访的92例肾移植受者为研究对象。采用一般情况调查表、免疫抑制剂治疗依从性Basel评估量表、感知健康能力量表、领悟社会支持量表、抑郁自评量表、药物不良反应症状表对受者进行测评。采用Logistic回归对肾移植术后服药依从性的影响因素进行分析。结果 :58.70%的肾移植受者服药依从性好;Logistic回归结果显示,性别[OR=0.333,95%?CI(0.131,0.850),P=0.021]、健康自我效能[OR=0.878,95%?CI(0.772,0.998),P=0.046]及领悟社会支持[OR=0.961,95%?CI(0.972,0.996),P=0.028]对老年肾移植受者术后的服药依从性有影响。结论 :老年肾移植受者术后的服药依从性欠佳。健康自我效能及社会支持会影响受者的服药依从性。Objective： To explore the current status and influencing factors of Immunosuppressive Medications adherence in older kidney transplant recipients. Methods： Totally 92 older kidney transplant recipients were recruited in the outpatient department in a tertiary Grade A hospital by convenience sampling method and investigated with the Demographic Questionnaire, Basel Assessment of Adherence with Immunosuppressive Medication Scale, Self-rating Depression Scale, Perceived Social Support Scale, Perceived Health of Control Scale, and Symptom Checklist. We analyzed the influencing factors of medication adherence among older kidney transplant recipients by logistic regression analysis. Results： The adherence rate was 58.70%. Logistic regression analysis showed that gender [OR=0.333, 95% CI（0.131, 0.850）, P=0.021], perceived health of control [OR=0.878, 95% CI（0.772, 0.998）, P=0.046] and perceived social support [OR=0.961, 95% CI（0.972, 0.996）, P=0.028] were influencing factors of medication adherence among older kidney transplant recipients. Conclusion： The medication adherence among older kidney transplant recipients is poor. Perceived health of control and perceived social support are predictors of medication adherence among older kidney transplant recipients.
