摘  要:目的探究高压氧治疗对新生儿期后缺氧缺血性脑病血清B淋巴细胞瘤-2(Bcl-2)蛋白的影响。方法随机选取2014年1月至2015年6月胜利油田中心医院儿科一病区收治的87例期后缺血缺氧性脑病患儿作为本次研究试验对象,根据入院时间先后将患儿分成观察组49例和对照组38例,对照组患儿采用常规治疗方案,观察组患儿在此基础上联合高压氧治疗,观察2组患儿治疗后临床疗效,记录并比较2组患儿治疗前、治疗7d、治疗10d后血清内Bcl-2和神经元特异性烯醇化酶(NSE)水平变化情况。结果2组患儿治疗前血清内Bcl-2水平相当,差异无统计学意义(P>0.05);治疗后观察组患儿Bcl-2水平下降快,且明显低于对照组患儿,差异有统计学意义(P<0.05),组内差异有统计学意义(P<0.05)。治疗前后2组患儿血清内NSE水平无明显变化,组间差异无统计学意义(P>0.05),组内差异也无统计学意义(P>0.05)。观察组显效患儿22例,总有效率为76%,对照组总有效率约为55%,2组差异有统计学意义(P<0.05)。结论高压氧治疗期后缺血缺氧性脑病患儿临床疗效显著,治疗后患儿血清内Bcl-2水平明显降低,改善脑循环,抑制神经细胞凋亡,促进脑细胞恢复,预后良好,但是治疗前后患儿血清内NSE没有明显变化。Objective To study the Hyperbaric Oxygen Therapy on Serum of Bcl-2 in the neonatal period after hypoxic-ischemic eneephalopathy. Methods Eight-seven cases of hypoxic-ischemic encephalopathy as test subjects in this study from January 2014 to June 2015 Shengli Oil Field Center Hospital pediatric ward of a trea- ted, according to the admission of children into chronological observation former group 49 cases and 38 cases in the control group, children in the control group with conventional treatment regimens, patients in the observation group on the basis of hyperbaric oxygen therapy, observed in children after two clinical efficacy, treatment records and compared two groups of children treatment 7 d, 10 d after treatment serum levels of Bcl-2 and NSE changes, analyzed and compared. Results Two groups of children within the first treatment serum levels of Bel-2 rather, the difference was statistically significant (P 〉0.05); observation group decreased after treatment in children with Bcl-2 levels quickly, and significantly lower than children in the control group, the difference was statistics significance ( P %0. 05), within the group also had statistically significant difference ( P %0.05). Within the two groups before and after treatment serum NSE levels had no significant change between the two groups was not sta- tistically significant ( P 〉0.05) differences within the group also was no significant difference ( P 〉0. 05). Chil- dren 22 cases markedly observation group, the total effective rate was 76 %, the total efficiency of control group a- bout 55%, the difference was statistically significant ( P 〈0.05). Conclusion Hyperbaric oxygen therapy on hy- poxic-ischemic encephalopathy significant clinical effect after treatment within the Bcl-2 levels decreased serum, improve cerebral circulation, inhibit neuronal apoptosis, promote brain cell recovery, the prognosis is good, but the treatment NSE no significant changes in the serum before and after.
