摘  要:目的:探讨右美托咪定对小鼠肺缺血/再灌注损伤的保护作用。方法:实验用雄性8~10周龄C57BL/6J小鼠50只,随机分为5组:假手术组(sham operation,sham组),缺血-再灌注组(I/R组),I/R+生理盐水组(NS组),I/R+右美托咪定组(DEX组),I/R+阿替美唑(Atipamezole,ATI)+DEX组(ATI+DEX组)。复制在体小鼠左肺I/R损伤模型,I/R组小鼠开胸夹闭左肺门缺血30 min,再灌注180 min;sham组仅开胸游离肺门而不夹闭,肺门游离后机械通气210 min;DEX组在左肺缺血前30 min腹腔注射DEX25μg/kg,NS组则腹腔注射同体积生理盐水;ATI+DEX组在缺血前30 min腹腔注射ATI250μg/kg后,随即腹腔注射DEX 25μg/kg。实验结束时取左肺组织测肺组织湿/干重比(W/D)、总肺含水量(TLW),评估肺组织损伤指数(IQA),光镜和电镜下观察肺组织的形态学变化。结果:与sham组比,其余4组肺组织W/D、TLW、IQA值明显上升(P〈0.05),光镜、电镜示肺组织呈明显损伤性变化;I/R组与NS组间上述指标差异无统计学意义(P〉0.05);DEX干预后,肺组织W/D、TLW、IQA值较I/R组明显降低(P〈0.01),肺组织损伤明显改善;应用阿替美唑以后,肺组织损伤加重,W/D、TLW、IQA值明显升高,与DEX组相比差异具有统计学意义(P〈0.01)。结论:右美托咪定可能通过抑制交感活性,改善缺血区域的血流,对缺血再灌注肺发挥保护作用。AIM: To explore the protection of Dexmedetomidine( DEX) on the lung ischemiareperfusion injury in mice. METHODS: 50C57 BL /6J male mice,aged 8-10 weeks,were randomly divided into 5 groups( n = 10) : sham operation group( sham group),lung ischemia-reperfusion group( I/R group),I/R + saline( NS group),I/R+ DEX group( DEX group),I/R + ATI + DEX group( ATI + DEX group). The lung ischemiareperfusion injury model in mice was replicated. In I/R group,the hilum of left lung was clamped for 30 min,followed by 180 min-reperfusion. For sham group,the hilum of left lung was only separated without clamping. DEX( 25 μg/kg) was infused intraperitoneally( i. p) 30 min prior to clamping in DEX group,NS group was infused( i. p) with the same volume normal saline,then followed by 180min-reperfusion period. ATI + DEX group was infused intraperitoneally( i. p) with ATI( 250 μg/kg,i. p.) prior to DEX infusion. After reperfusion,the left lung tissues were harvested to determine the lung wet/dry weight( W/D),the total lung water content( TLW),and index of quantitative evaluation for alveolar damage( IQA). The morphological changes of lung tissue were observed under light microscope and electronmicroscope. RESULTS: Compared with the sham group,the values of W/D,TLW,IQA were obviously raised( P 〈0. 05),light microscope and electron microscope showed serious morphological and ultrastructure injury in the other four groups;the above index in I/R group and NS group showed no obvious differences( P 〈0. 05); compared with the I/R group,the values of W/D,TLW,IQA were significantly decreased in the DEX group( P 〈0. 01),meanwhile,light morphological and ultrastructure injury was found to be alleviated; in ATI +DEX group,the lung injury became more aggravated than that in the DEX group,the values of W/D,TLW,IQA were significantly higher than those in the DEX group.
