摘　　要：目的：分析甲状腺围手术期患者预防用抗菌药物的合理性，为进一步规范抗菌药物的临床使用提供参考。方法：选取2015年1月-2015年5月间收治的甲状腺手术患者702例病历，分析预防性使用抗菌药物情况，包括抗菌药物使用率、药物种类、用药时机、用药疗程等。结果：702例围手术期患者中，其中使用抗菌药物203例，其使用率为28．92％；使用抗菌药物的203例病例中，术前0．5-2h给药的为191例（94．09％），术前未用术后用患者为12例（5．91％）；术前使用术后未用患者为21例（10.35％）；术后用药时间小于2d的为91例（44．83％），2-3d的为12例（5．91％），3-7d的为70例（34．48％），大于7d的为9例（4．43％）；无指征用药为23例（11.33％），药物选择合理的为198例（97．53％）。结论：该院甲状腺围手术期患者合理使用抗菌药物的成效较为显著，但仍有不足之处，尚待进一步加强监管。Objective： To analyze the clinical medication of perioperative antibiotics in 702 patients with thyroid and to provide a reference for the rational of use of antibiotics. Methods： The records of thyroid surgery patients admitted from Mar. 2015 to May 2015 were selected. The perioperative use of prophylactic antibiotics was analyzed retrospectively, including the utilization rates of antibiotics, types, medication timing and duration. Results： In the 702 patients with thyroid, 203 patients used antibiotics and the percentage of antibiotics use was 28.92%. In the 203 cases who used antibiotics, the number of using antibiotics 0.5 to 2 h before operation was 191 cases（94.09%）. 12 cases（5.91%） did not use antibiotics preoperatively and used antibiotics postoperatively. 21 cases （10.35%） used antibiotics preoperatively and not used antibiotics postoperatively. 91 cases（44.83%） used antibiotics for less than 2d and 12 case（5.91%） at 2 to 3d and 70 cases（34.48%） at 3 to 7 d and 9 cases（4.43%） more than 7d after operation. 23 cases（11.33%） used antibiotics without indication. The rational choice of drug variety was 198 cases（97.53%）. Conclusion： The effects of perioperative antibiotics in patients with thyroid were more significant, but there are still some shortages which remained to be overcome, and measures should be taken to strengthen control on antibiotic use.
