皮下通道型胆囊肝胆管成型术治疗肝胆管结石合并感染的临床效果分析

摘　　要：目的分析皮下通道型胆囊肝胆管成形术治疗肝胆管结石合并感染的临床效果,为临床治疗肝胆管结石合并感染提供参考依据。方法选取2011年1月-2013年12月108例在医院治疗的肝胆管结石合并感染患者,分为观察组与对照组,各54例;对照组使用传统手术治疗,观察组使用皮下通道型胆囊肝胆管成形术治疗,观察两组治疗有效率,数据采用SPSS 16.0软件进行统计分析。结果观察组患者治疗有效率为92.59%,对照组为77.78%;观察组患者并发症发生率为12.96%,低于对照组的53.70%;于1年后对患者进行随访发现,观察组复发率为9.26%;对照组复发率为22.22%,两组比较,差异均有统计学意义(P<0.05)。结论皮下通道型胆囊肝胆管成形术具有较高的临床应用效果,可有效的减少患者在术后出现并发症的发生率,同时其复发率较低,值得在临床中推广使用。OBJECTIVE To observe the clinical effect of subcutaneous tunnel gallbladder bile duct angioplasty on treatment of hepatolithiasis complicated with infections so as to provide guidance for clinical treatment of hepatolithiasis complicated with infections.METHODS A total of 108 hepatolithiasis patients complicated with infections were enrolled in the study and divided into the observation group and the control group,with 54 cases in each group.The control group was treated with conventional surgical procedures,while the observation group was given subcutaneous tunnel gallbladder bile duct angioplasty.The effective rate of treatment was observed and compared between the two groups,and the statistical analysis of data was performed with the use of SPSS 16.0software.RESULTS The effective rate of treatment was 92.59% in the observation group,77.78% in the control group;the incidence of complications was 12.96% in the observation group,lower than 53.70% in the control group.The one-year follow-up showed that the recurrence rate was 9.26%in the observation group,22.22%in the control group,and there was significant difference between the two groups（P0.05）.CONCLUSION The subcutaneous tunnel gallbladder bile duct angioplasty can achieve significant clinical effect and effectively reduce the incidence of postoperative complications,with the recurrence rate low,and it is worthy to be promoted in the hospital.
