摘 要:目的探讨纤维支气管镜与肺泡灌洗术治疗老年重症肺部感染临床疗效,为临床治疗该类疾病提供依据。方法选取2011年1月-2015年1月治疗的老年重症肺部感染患者86例,随机分为观察组和对照组,每组各43例;对照组患者接受常规治疗,观察组患者在常规治疗基础上加用纤维支气管镜肺泡灌洗术治疗;观察记录两组患者治疗效果、临床体征恢复时间及不良反应发生率;数据采用SPSS 19.0软件进行统计分析。结果观察组患者咳嗽消失时间、咳痰消失时间、肺部啰音消失时间和住院时间均少于对照组患者,两组之间差异有统计学意义(P<0.05);患者治疗总有效率观察组为90.70%,对照组为65.12%,差异有统计学意义(P<0.05);两组患者动脉血气指标氧分压(PaO2)、二氧化碳分压(PaCO2)和血氧饱和度(SpO2)灌洗后有明显改善,差异有统计学意义(P<0.05),但观察组改善更为明显,与对照组相比,差异有统计学意义(P<0.05)。结论纤维支气管镜和肺泡灌洗术治疗老年重症肺部感染能明显提高疗效,缩短住院时间。OBJECTIVE To explore the clinical efficacy of fiberoptic bronchoscopy and bronchoalveolar lavage in treatment of elderly patients with severe pulmonary infections so as to provide guidance for clinical treatment of the disease.METHODS A total of 86 elderly patients with severe pulmonary infections who were treated from Jan 2011 to Jan 2015 were enrolled in the study and randomly divided into the observation group and the control group,with43 cases in each.The control group was given the conventional treatment,while the observation group was treated with additional fiberoptic bronchoscopy and bronchoalveolar lavage based on the conventional treatment.The therapeutic effect,time of recovery of clinical signs,and incidence of adverse reactions were observed and recorded,and the statistical analysis of data was performed with the use of SPSS 19.0software.RESULTS The time of disappearance of cough,time of disappearance of expectoration,time of disappearance of pulmonary rales,and length of hospital stay were significantly shorter in the observation group than in the control group(P0.05).The total effective rate of treatment of the observation group was 90.70%,significantly higher than 65.12% of the control group(P0.05).The partial pressure of arterial oxygen(PaO2),partial pressure of arterial carbon dioxide(Paco2),and saturation of peripheral blood oxygen(SpO2)of the two groups of patients were significantly improved after the lavage(P0.05);the improvement was more significant in the observation group than in the control group(P0.05).CONCLUSION The fiberoptic bronchoscopy and bronchoalveolar lavage can remarkably improve the clinical effect on treatment of the elderly patients with severe pulmonary infections and shorten the length of hospital stay.
