男性染色体多态性对体外受精结局的影响

摘　　要：目的:研究男性染色体多态性对体外受精(in vitro fertilization,IVF)/卵胞浆内单精子注射(intracytoplasmic sperm injection,ICSI)治疗结局的影响及男性染色体多态对精液质量的影响。方法:根据2012年6月至2013年6月在我院行试管助孕治疗的男性染色体检查结果分为试验组(41例)及对照组(124例),回顾性分析了两组IVF/ICSI妊娠结局及两组精液质量。结果:(1)试验组IVF受精率(71%vs 87%,P=0.001)、卵裂率(75%vs 89%,P=0.003)、优胚率(51%vs 66%,P=0.049)、临床妊娠率(14%vs62.5%,P=0.033)均较对照组低,差异有统计学意义,着床率、生化妊娠率、早期流产率相似,无统计学意义。(2)试验组ICSI优胚率(52%vs 59%,P=0.047)较对照组低,其余统计数据均无统计学差异。(3)试验组精液质量正常率较对照组低,差异有统计学意义。总结:男性染色体多态性对IVF有不利的影响,对ICSI妊娠结局影响不大。男性染色体多态性会降低精液质量。Objectives： To assess the effect of male chromosome polymorphism on in vitro fertilization（ IVF） /intracytoplasmic sperm injection（ ICSI） reproductive outcome and assess the effect of male chromosome polymorphism on semen quality. Methods： The patients were divided into experiment group（ 41 patients） and control group（ 124 patients） according to chromosome results. The reproductive outcome and semen quality of the two groups were retrospectively analyzed. Results： ①In experiment group,IVF fertilization rate（ 71% vs. 87%,P =0. 001）,cleavage rate（ 75% vs. 89%,P = 0. 003）,good embryo rate（ 51% vs. 66%,P = 0. 049）,clinical pregnancy rate（ 14% vs. 62. 5%,P = 0. 033） were all lower than these in control group,and there were statistically significant differences. There were no significant difference in implantation rate,biochemical pregnancy rate and early miscarriage rate. ②In experiment group,ICSI good embryo rate（ 52% vs. 59%,Pv = 0. 047） was lower than that in control group. There was no significant difference in other statistical data. ③Semen quality in experiment group was lower. Conclusions： Male chromosome polymorphism has adverse effect on IVF reproductive outcome and has no obvious effect on ICSI reproductive outcome. Male chromosome polymorphism can reduce semen quality.
