降钙素原在血流感染及局部细菌感染中的诊断作用

摘　　要：目的探讨降钙素原（PCT）和超敏C反应蛋白（hs-CRP）定量检测在血流感染及局部细菌感染中的临床应用价值,为感染性疾病的早期诊断提供依据。方法采用回顾性研究,对本院临床确诊的97例细菌血流感染及103例局部细菌感染者的PCT、hs-CRP值进行分析。比较PCT、hs-CRP浓度在血流感染和局部细菌感染之间的差异,并与80例正常人的检测结果比较。结果血流感染患者PCT水平为5.59（1.075~23.46）ng/m L,hs-CRP为85.3（55.4~147.7）mg/L;局部感染患者PCT水平为0.15（0.09~0.35）ng/m L,hs-CRP为37.5（15.3~75.3）mg/L。血流感染组、局部细菌感染组及正常对照组三者的PCT、hsCRP比较差异有统计学意义（P〈0.05）。PCT、hs-CRP诊断血流感染与局部细菌感染的受试者工作特征曲线下面积（ROC-AUC）分别为0.905、0.745。结论 PCT、hs-CRP都可用于血流感染和局部细菌感染的早期鉴别诊断,但是PCT的诊断价值更高。Objective To explore the clinical value of procalcitonin（ PCT） and high sensitive C-reactive protein（ hs-CRP） in diagnosis of bloodstream infection and localized bacterial infection,and provide a reference for the early diagnosis of infectious diseases. Methods Retrospective analysis was used to observe the serum level of PCT and hs-CRP in 97 bloodstream infection cases and 103 localized bacterial infection cases from our hospital. The difference of PCT and hs-CRP levels among the patients with bloodstream and localized infection and 80 healthy people were analyzed. Results The serum levels of PCT and hs-CRP in bloodstream infection patients were 5. 59（ 1. 075- 23. 46） ng / m L and 85. 3（ 55. 4- 147. 7） mg / L,respectively. In localized bacterial infection patients,they were 0.15（ 0.09- 0.35） ng / m L and 37.5（ 15.3-75.3） mg / L,respectively. There was a statistical difference in the serum level of PCT and hs-CRP among the patients with bloodstream and localized bacterial infection and healthy people（ P〈0. 05）. The areas under the receiver-operating characteristic curve（ ROC-AUC） in diagnosis of bloodstream infection and localized bacterial infection were 0.905 for PCT and 0.745 for hs-CRP. Conclusion PCT and hs-CRP could be used in early diagnosis of bloodstream infection and localized bacterial infection,in which PCT is better.
