摘  要:[背景]探讨外侧小切口人工股骨头置换术治疗不稳定型股骨颈骨折的临床疗效.[病例报告]施行外侧小切口入路人工股骨头置换术治疗不稳定型股骨颈骨折76例,其中Garden分型Ⅱ型4例,Ⅲ型49例,Ⅳ型23例;Pauwels角为55°~83°,平均67.2°;术前Harris评分为32~57分,平均42.6分.手术切口长8~12cm,平均9.8cm;手术时间85~115min,平均92min;术中出血量为235~670mL,平均381mL;术后引流量为140~235mL,平均209mL;术后随访14~18个月,平均16.7个月.术后1年Harris评分平均为93.3分,未见切口感染等并发症发生.[讨论]外侧小切口人工股骨头置换术治疗不稳定型股骨颈骨折手术切口短,创伤小,出血少,术后功能恢复快,关节恢复情况满意.BACKGROUND To study the curative effects of mini-invasive bipolar hemiarthroplasty through lateral approach on treatment of unstable femoral neck fracture.CASE REPORTS76 patients with unstable femoral neck fracture were treated by mini-invasive bipolar hemiarthroplasty through lateral approach,in which 4patients belonged to Garden typeⅡ,49 patients Garden typeⅢ and 23 patients Garden type Ⅳ.Pauwels angle was from 55°to 83°(average 67.2°),and the preoperative Harris score was from 32 to 57(average 42.6).Average incision length was 9.8(8-12)cm,average operation time was 92(85-115)min,average intraoperative blood loss was 381(235-670)mL,and average drainage was 209(140-235)mL.All the patients were followed up for 14-18month(mean 16.7month),and the average Harris score was 93.3after operation for 1year,and there were no complications of incision infection,and so on.DISCUSSION Mini-invasive artificial hemiarthroplasty through lateral approach for treatment of unstable femoral neck fracture has the advantages of short operative incision,small wound,reducing blood loss,accelerating postoperative rehabilitation and satisfied postoperative recovery of joint function.
