摘  要:目的研究加味真武汤对阿霉素诱导的大鼠心衰模型的保护作用并探讨其机制。方法尾静脉注射阿霉素建立心衰模型,于1、3、5周注射阿霉素3 mg/kg,于2、4、6周尾静脉注射阿霉素2 mg/kg,共注射6次。60只大鼠随机分为6组,每组10只,分别为空白对照组,模型组,加味真武汤高、中、低剂量组(40、20、10 g生药/kg,灌胃),氢氯噻嗪(阳性药物)组(10.78 mg/kg,灌胃)。观察动物一般状况,24 h尿量、尿液中Na+、K+及Cl-含量,比较各组心功能,血浆中血管紧张素(angiotensin,AngⅡ)、醛固酮(aldosterone,ALD)、抗利尿激素(antidiuretic hormone,ADH)含量的变化。大鼠处死后取心脏,常规HE染色,进行病理组织学检查。结果给予加味真武汤后,给药组大鼠摄食、活动等较未给药前改善。与模型组相比,加味真武汤高、中、低剂量组24h尿量、尿液中Na+及K+含量均不同程度增加(P<0.05),但Cl-含量无显著变化,降低左心室舒张末期压力(left ventricular end dilated pressure,LVEDP),升高左室内压峰值(left ventricular systolic pressure,LVSP)、左室压力上升最大速率(+dp/dtmax)及左室压力下降最大速率(-dp/dtmax)(P<0.05),降低AngⅡ、ADH及ALD水平(P<0.05)。病理结果显示,模型组心肌细胞明显受损并有单核、淋巴细胞浸润,高剂量组、阳性组心肌细胞基本恢复正常,中、低剂量组有一定程度的改善。结论加味真武汤改善心衰大鼠的心功能,此作用可能与其降低AngⅡ、ALD、ADH水平,促进Na+、K+排泄,从而增加尿量有关。Objective To explore the protective effects of Jiawei Zhenwu decoction on rats with heart failure induced by adriamycin and its mechanism. Methods The heart failure was established with adriamycin by tail intravenous injection. 3 mg / kg of adriamycin was injected on 1st,3rdand5 thweek and 2 mg / kg of adriamycin was injected on 2nd,4thand 6thweek,a total of 6 times. 60 rats were randomly divided into six groups,10 rats in each group: blank control group,model group,low-,middle-and high-does Jiawei Zhenwu decoction( 40,20,10 g / kg,intragastric administration),and hydrochlorothiazide group( positive drug group,10. 78 mg / kg,intragastric administration). The overt behavior was observed and 24 h urine volume,Na+,K+and Cl-content in urine,cardiac function and levels of angiotensin( AngⅡ),aldosterone( ALD) and antidiuretic hormone( ADH) in plasma were compared among each group. After the rat were sacrificed,the heart was taken out and stained by HE to analyse histopathologic examination.Results The spontaneous activity and ingestion of rats improved after given Jiawei Zhenwu decoction. The 24 h urine volume,Na+,K+content in urine increased of low-,middle- and high-does Jiawei Zhenwu decoction at different degrees( P〈0. 05),while there were no significant changes of Clcontents; left ventricular end dilated pressure( LVEDP) decreased,left ventricular systolic pressure( LVSP), + dp / dtmaxand-dp / dtmaxincreased in low-,middle-and high-does Jiawei Zhenwu decoction at different degrees( P〈0. 05); the plasma levels of angiotensin( AngⅡ),aldosterone( ALD) and antidiuretic hormone( ADH) decreased( P〈0. 05). The pathological examination result showed that there was significant damage of myocardial cell and infiltration of monocyte and lymphocyte,myocardial cell of high- dose group almost returned to normal and middle-and low-dose group have improvement to some extent.
