体力活动咨询对稳定期抑郁症患者体力活动水平的影响

摘  要:目的研究体力活动咨询对稳定期抑郁症患者体力活动的影响,为提高临床治疗效果提供依据。方法选取2014年6-12月中山大学附属第三医院精神科门诊48例康复期抑郁症患者为研究对象,随机分为体力活动咨询组和对照组,每组24例,其中男性20例,女性28例,年龄22~55岁,平均(34.8±7.9)岁;病程4~18个月。对照组予以简单的"增加体力活动"的建议,体力活动咨询组予以体力活动咨询,基线、2周末、4周末、6周末各访谈1次,评价指标包括中、高强度体力活动次数、时间,汉密尔顿抑郁量表、匹兹堡睡眠质量指数(PSQI)和疲劳量表-14(FS-14),基线和6周末各评定1次。应用SPSS 16.0软件对数据进行χ2检验及t检验。结果 6周末,体力活动咨询组中等强度体力活动次数[(3.79±0.88)次、时间[(127.92±43.13)min/周]、高强度体力活动时间[45(0,60)min/周]、增加体力活动的信心[(6.45±1.62)分)]和意愿[(7.74±3.33)分]均高于对照组[分别为(2.13±1.09)次、(64.00±38.67)min/周、0(0,35)min/周、(4.02±1.95)分和(5.63±2.83)分],差异均有统计学意义(P〈0.01,P〈0.05);体力活动咨询组FS-14和PSQI量表评分[分别为(3.08±1.33)和(3.83±1.30)分]低于对照组[分别为(5.88±1.31)和(4.46±1.53)分],差异均有统计学意义(P〈0.05,P〈0.01)。结论体力活动咨询可有效提高稳定期抑郁症患者的体力活动水平,并且有助于改善其失眠情况,值得推广。Objective To study the effect of physical activity(PA) consulting on the patients with stable major depression and to provide the basis for improving the clinical treatment. Methods A total of 48 patients with major depression were selected as the subjects(20 males and 28 females) during June to December of 2014. The subjects were randomly divided into physical activity consulting group(24 cases) and control group(24 cases). Average age was(34.8±7.9) years old(22-55 years old). The course of disease was 4-18 months. The control group only received the simple suggestion of increasing PA, the physical activity consulting group received the PA consulting. All subjects were interviewed in 0, 2, 4 and 6 weeks after consulting. The assessment indexes included the high PA times and duration, Hamilton Depression Scale( HAMD- 24), Pittsburgh Sleep Quality Index(PSQI) and Fatigue Scale(FS-14). The assessment was performed in 0 and 6th week after consulting. χ2test and t test were used to analyze all data with SPSS 16.0 software. Results After 6 weeks of consulting, the times and duration of moderate PA(3.79±0.88 times and 127.92 ±43.13 min/week), the duration of high PA [45(0,60) min/week], confidence score of PA(6.45 ±1.62scores) and aspiration score of PA(7.74±3.33 scores) in PA consulting group were significantly higher than those(2.13±1.09 times, 64.00±38.67 min/week, 0(0,35) min/week, 4.02±1.95 scores and 5.63±2.83 scores) in control group(P〈0.05 or P〈0.01);the scores(3.08±1.33 and 3.83±1.30) of FS-14 and PSQI in PA consulting group were significantly lower than those(5.88±1.33 and 4.46 ±1.53) in control group(P〈0.05 or P〈0.01). Conclusion PA consulting is effective for increasing the PA levels and helpful for improving the sleep quality in patients with major depression.
