PFNA-Ⅱ与PCCP应用于高龄A2型股骨粗隆外侧壁骨折的疗效比较

摘　　要：目的对比股骨近端髓内钉-螺旋刀片Ⅱ型（PFNA-Ⅱ）与经皮加压钢板（PCCP）治疗高龄A2型股骨粗隆外侧壁骨折的疗效。方法回顾分析2010年1月~2014年6月收治并符合标准的77例高龄A2型股骨粗隆外侧壁骨折病人资料,采用PFNA-Ⅱ治疗的患者42例,PCCP治疗的患者35例,比较两组手术的手术时间、切口长度、术中总失血量、术后下地活动时间、透视次数、Harris评分指标差异。结果 PFNA-Ⅱ手术时间、切口长度、术后下地活动时间均优于PCCP组（P〈0.05）,但术中透视次数多于PCCP组（P〈0.05）;两组患者均获随访,PFNA-Ⅱ组随访时间10~24个月,PCCP组12~24个月。两组术中总出血量、术后12个月Harris评分比较差异无统计学意义（P〉0.05）。结论 PFNA-Ⅱ与PCCP治疗高龄A2型股骨粗隆外侧壁骨折都具有良好的疗效,但PFNA-Ⅱ具有骨折愈合率高、早期下地、功能恢复佳的优势。Objective To compare the effectiveness of the proximal femoral nail anti-rotation Ⅱ（ PFNA-Ⅱ） and percutaneous compression plate（ PCCP） in the treatment of A2 type lateral femoral trochanter wall fracture in elderly patients. Methods From Jan 2010 to Jun 2014,77 elderly patients with A2 type lateral femoral trochanter wall fracture were treated. Fractures were fixed with PFNA-Ⅱin 42 patients（ PFNA-Ⅱ group）,and with PCCP in 35 patients（ PCCP group）. Two groups of surgical operation time,incision length,intraoperative blood loss,time for ambulation,number of perspective and Harris hipscore were compared. Results The surgical operation time,incision length and time for ambulation of PFNA-Ⅱ group were significantly better than those of the PCCP group（ P〈0. 05）,but the number of perspective of PFNA-Ⅱ group was significantly greater than that of PCCP group（ P〈0. 05）. Two groups of patients were followed up,PFNA- II patients were followed up for 10 ~ 24 months,PCCP group were for 12 to 24 months. There was no significant difference in total blood loss and Harris hipscore between the two groups in 12 months after operation（ P〈0. 05）. Conclusion Both PFNA-Ⅱ and PCCP have a good effect in treatment of A2 type lateral femoral trochanter wall fracture in the elderly patients,by contrast,PFNA-Ⅱ has a high rate of fracture healing,early fields,good functional recovery.
