单操作孔胸腔镜在创伤性血气胸中的应用

摘　　要：目的分析单操作孔胸腔镜与传统剖胸术在治疗胸部创伤性血气胸中的临床疗效,为胸部创伤的救治提供可靠依据。方法 2012年10月-2015年2月陕西省榆林市第一医院胸心外科收治胸部创伤性血气胸患者86例,随机分为治疗组(n=43)和对照组(n=43);治疗组采用单操作孔胸腔镜手术,对照组采用传统剖胸手术,比较两组术中出血量、手术时间、术后第3天疼痛评分、术后住院时间。结果两组患者性别、年龄、临床诊断类型差异无统计学意义(P>0.05)。治疗组术中出血量、手术时间、术后第3天疼痛评分、术后住院时间明显优于对照组(P<0.05)。结论单操作孔胸腔镜手术治疗胸部创伤性血气胸,安全有效、创伤小、并发症少。Objective To analyze the clinical efficacy of single port thoracoscopy and traditional cesarean section surgery in the treatment of chest trauma hemopneumothorax, and provide a reliable basis for treatment of chest trauma. Methods Eighty-six patients with chest traumatic hemopneumothorax admitted to the department of chest cardiac, First Hospital in Yulin City of Shaanxi province from October 2012 to February 2015 were enrolled in this study and they were randomly divided into treatment group（n=43） and control group（n = 43）. Patients in treatment group underwent single port thoracoscopic operation, while others in control group underwent traditional cut open chest surgery. The intraoperative blood loss, operative time, pain score at the third day after operation and length of hospital stay of the two groups were compared. Results There were no statistically significant differences in gender, age and clinical diagnosis of type between patients in two groups（P〉0.05）. The intraoperative blood loss, operative time, pain score at the third day after operation and length of hospital stay of treatment group were better than control group with significant differences（P〈0.05）. Conclusion Single port thoracoscopic surgery in the treatment of thoracic traumatic hemopneumothorax operation is safe and effective with small trauma and fewer complications, which is better than the conventional thoracic surgery and worthy of clinical application.
