牡甲化纤方对熊去氧胆酸应答不佳的原发性胆汁性肝硬变患者免疫功能的影响

摘　　要：目的:探讨牡甲化纤方对熊去氧胆酸应答不佳的原发性胆汁性肝硬变患者肝功能及T淋巴细胞亚群、细胞因子IL-2、TNF-α的影响。方法:将60例熊去氧胆酸应答不佳的原发性胆汁性肝硬变患者分为治疗组和对照组,每组30例。治疗组给予牡甲化纤方治疗,对照组给予苯扎贝特治疗,观察治疗前后两组患者肝功能及T淋巴细胞亚群、细胞因子IL-2、TNF-α的变化。结果:治疗组总胆红素、碱性磷酸酶、谷草转氨酸、谷氨酸转移酶均较治疗前明显降低,差异有显著性(P<0.05);与对照组治疗后比较,差异具有统计学意义(P<0.05);疗程结束后治疗组CD4^+水平下降,CD8^+水平上升,CD4^+/CD8^+比值下降,同时IL-2及TNF-α水平也较治疗前下降,差异有显著性(P<0.05);与对照组治疗后比较,治疗组治疗后CD4^+、CD8^+、CD4^+/CD8^+、IL-2、TNF-α变化差异更有显著性(P<0.05)。结论:牡甲化纤方对熊去氧胆酸应答不佳的原发性胆汁性肝硬变患者疗效良好,可能与该方能够调节机体免疫功能紊乱有关。Objective： To explore the influence of Mu Jia Hua Xian Decoction（ MJHXD） on liver function,lymphocyte subsets,interleukin-2 and tumor necrosis factor-αof patients with primary biliary cirrhosis（ PBC） with a poor response to ursodeoxycholic acid. Methods： Sixty patients with PBC with a poor response to ursodeoxycholic acid were randomly divided into the treatment group and the control group,with 30 cases in each group. The patients in the treatment group were added MJHXD,while the ones in the control group were added bezafibrate. Then the changes of liver function,lymphocyte subsets,interleukin-2 and tumor necrosis factor-αof patients in both groups before and after the treatment were observed. Results： TBIL,AST,ALP and GGT of the treatment group after the treatment were all lower than those before the treatment; And the differences between them had significance（ P 〈 0. 05）; Compared with those of the control group after the treatment,differences between them had statistical meaning（ P 〈0. 05）. After the medical course was over,in the treatment group,CD4~＋level decreased,CD8~＋level improved,CD4~＋/ CD8~＋ratio decreased; At the same time,IL-2 level and TNF-αlevel decreased compared with before the treatment; And the differences between them had significance（ P 〈 0. 05）. Compared with the control group after the treatment,in the treatment group,the change differences in CD4~＋,CD8~＋,CD4~＋/ CD8~＋,IL-2,and TNF-α had more significance（ P 〈 0. 05）. Conclusion： MJHXD has good curative effect on patients with PBC with a poor response to ursodeoxycholic acid,which probably has something to do with its function of regulating disorder of body’s immune function.
