杨晋翔通降法治疗胆汁反流性胃炎经验

摘　　要：杨晋翔教授认为本病病位在胆胃两腑,与肝、脾关系密切,病性属于本虚标实,本虚指脾胃气虚,标实则为痰热、气滞、血瘀、湿热等。"胆胃不和"是导致胆汁反流性胃炎发生发展的基本病机,提出本病治法"主在胆胃,贵在通降"。杨晋翔教授常用治法有:清胆和胃通降法、理气化瘀通降法、健脾和胃通补法、养阴益胃通降法及结合胃镜表现用药。杨老师临证处方,善用药对,在胆汁反流性胃炎治疗中"见一症用一对药",其常用药对有:旋覆花、代赭石;黄连、吴茱萸;川楝子、延胡索;香附、苏梗、陈皮。Professor Yang Jinxiang believes that the location of this disease is closely related with the gallbladder,stomach,liver and spleen. The nature of the disease is deficient root with excess superficial. Deficient root refers to the stomach deficiency while excess superficial means phlegm heat,qi stagnation,blood stasis,damp heat,etc. ＂ Disharmony between gallbladder and stomach＂ is the basic pathogenesis of the occurrence and development of bile reflux gastritis（ BRG）. He proposes the treatment should be focused on the gallbladder and the stomach with the method of dredging and draining downward. The commonly-used methods he applies include dredging method of clearing gallbladder and harmonizing stomach,dredging method of regulating qi and resolving blood stasis,dredging and tonifying method of strengthening spleen and harmonizing stomach,dredging method of nourishing yin and benefiting stomach combined with drugs used according to the gastroscopy. In clinical practice Professor Yang is good at applying medicine pair. In the treatment of BRG,a pair of medicine are used corresponding to a symptom. The common medicine pairs include Xuanfuhua（ Flos Inulae）,Daizheshi（ Haematitum）,Huanglian（ Rhizoma Coptidis）,Wuzhuyu（ Fructus Evodiae）,Chuanlianzi（ Fructus Toosendan）,Yanhusuo（ Rhizoma Corydalis）,Xiangfu（ Rhizoma Cyperi）,Sugeng（ Caulis Perillae） and Chenpi（ Pericarpium Citri Reticulatae）.
