摘  要:目的通过对计算机辅助下的全膝关节置换术与传统膝关节置换术在术后1年的近期疗效对比,探讨计算机辅助下的膝关节置换术的优势。方法 2013年9月至2014年9月将60例患者分成两组,分别进行计算机辅助下的全膝关节置换术(导航组)和传统的全膝关节置换术(传统组)。并对每例患者进行术前、术后1年的KSS评分、WOMAC评分、Oxford评分、下肢全长X线片进行统计分析。结果患者随访时间12~14个月,平均12个月,导航组的下肢机械轴为-0.033°±1.470°,传统组的下肢机械轴为0.600°±2.989°;获得3°以内的内外翻力线的比例分别为86.70%和73.30%,术后KSS评分两组分别为(88.80±3.71)分和(82.63±4.15)分;WOMAC评分为(23.57±3.64)分和(30.00±4.89)分、Oxford评分为(18.53±3.66)分和(21.83±3.99)分,上述4项数据两组间差异均有统计学意义(P〈0.05)。结论通过计算机辅助下的全膝关节置换术,可获得更精确的下肢力线,且临床康复效果更具有优势。Objective To explore the advantages of computer assisted total knee arthroplasty(TKA)by comparing the postoperative 1year short term effect of the computer assisted TKA and the conventional TKA.Methods A total of 60 patients from September 2013 to September 2014 were randomly divided into two groups and performed TKA by adopting the computer-assisted technology and the conventional technology respectively.The KSS score,WOMAC score,Oxford score and long-leg weight-bearing X-ray before operation and at postoperative 1year were performed for each case and the statistical analysis was conducted.Results All cases were followed up for an average of 12months(12-14months).The mechanical limb axis in the navigation assisted group was-0.033°±1.470°,which in the traditional group was 0.600°±2.989°(t=-1.711,P〈0.05);the proportions of within 3°varus/valgus force lines were 86.70% and 73.30%respectively,the KSS scores were 88.80±3.71 and 82.63±4.15;the postoperative WOMAC scores were 23.57±3.64 and 30.00±4.89respectively;the Oxford scores were 18.53±3.66 and 21.83±3.99,the differences between the two groups were statistically significant(P〈0.005).Conclusion TKA by computer navigation guidance can obtain more accurate lower limb force lines,moreover the navigation group has more advantages in the clinical rehabilitation effect.
