摘 要:目的探讨学龄期儿童注意缺陷多动障碍(ADHD)社会功能与行为问题之间的关系。方法选取2013年9月~2014年9月到汕头大学精神卫生中心儿童精神科门诊首诊并符合美国精神障碍诊断与统计手册第5版(DSM-5)的学龄期ADHD儿童62例作为ADHD组,在公立小学中随机抽取77例学生作为对照组,两组家长均完成Conners父母行为问卷(PSQ)、儿童行为量表(CBCL)和Weiss功能缺陷量表(WFIRS-P)。结果与对照组比较,ADHD组PSQ的品行问题、学习问题、心身障碍、冲动-多动、焦虑、多动指数分值均高于对照组(P〈0.05);CBCL的行为分值高于对照组,而活动能力、社交能力、学习能力、社会能力总分均低于对照组(P〈0.001);WFIRS-P的家庭、学习和学校、生活技能、自我管理、社会活动、冒险活动和总分分值均高于对照组(P〈0.01),且与PSQ和CBCL的行为问题分值存在正相关(P〈0.05),与CBCL的活动能力、社交能力、学习能力和社会能力总分存在负相关(P〈0.01)。结论 ADHD儿童较正常儿童存在更多的行为问题和功能损害,且两者密切相关,临床治疗ADHD应重视儿童社会功能的康复。Objective The aim of this study was to explore the relationships between the social function and behavior problems in children with attention- deficit / hyperactivity disorder( ADHD). Methods Totally 62 ADHD children who met the diagnostic criteria of Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition( DSM- 5) for ADHD,came from the out- patient clinic of child psychistry in Shantou University Mental Health Center from September 2013 to September2014,and 77 normal children who came from the public primary schools were recruited in this study. The parents were asked to complete the Conners Parent Rating Scales( PSQ),Child Behavior Checklist( CBCL) and the Weiss Functional Impairment Scale- Parent( WFIRS- P). Results The results of PSQ showed that there were significant higher scores of conduct problem,learning problem,psychosomatic,hyperactivity and Conners Index of Hyperactivity( CIH) in the ADHD children than the normal children( P 〈0. 05). The behavioral scores of CBCL were also significant higher than the controls,while there were less ability in the ADHD children than the normal children. Comparing with the control group,children with ADHD had higher scores on the family,learning and school,life skills,child's self- concept,social activities,risky activities and total score in WFIRS- P. In ADHD children,the functional impairment had some positive relations to the behavior problems,while had some negative relations to the activity ability,social acceptability and learning ability of the CBCL( P 〈0. 01).Conclusion There were more behavior problems and functional impairment in the children with ADHD than the controls,and interacting with each other. Our findings suggest that interventions for ADHD should be perform to improve the child's functional level.
