医院用血合理性分析及应对措施的临床研究

摘　　要：目的：了解医院临床合理用血情况，分析不合理原因，提出相应改进措施和建议。方法：抽取我院2013年8月至2014年7月间输血病历3855份、输血次数4913次进行合理性评估。结果：血浆合理输血比率最低（占63％）；红细胞用量最大，合理输血比率为71．7％；冷沉淀合理输血比率为92％，血小板合理输血比率为100％。结论：临床用血不合理现象存在，建议加强输血软件的规范化管理，加大输血相关知识的培训力度，用血科室确定专职质控员督导检查科室内部临床用血合理性，健全医院质量评价考核体系，将合理用血与科室绩效考核和医生个人业绩考核挂钩，促进临床规范、科学、合理用血。Objective ： To investigate the current situation of blood transfusion, to analysis the reasons of unreasonable blood transfusions and to develop corresponding improvement measures. Methods ： We collected 3855 medical records and evaluated the rationality of 4913 blood transfusions documented in the records.Results ： The rates of reasonable blood transfusion for plasma, red blood cell,cold precipitation and platelet were 63.0%, 71.7%, 92.0% and 100.0%, respectively. ConclusJorls ： There was some unreasonable phenomenon in clinical blood transfusion. Strengthening the standard management of blood transfusion software, improving the awareness of knowledge about blood transfusion, establishing a sound hospital quality assessment system related to the blood transfusion were the effective measures to promote the standard, scientific and rational use of blood transfusion.
