摘  要:目的探讨孕酮对大鼠臂丛神经损伤后轴突再生及感觉功能恢复的影响。方法建立大鼠臂丛神经损伤动物模型,24只大鼠随机分为3组(每组8只):盐水组、孕酮溶剂组(以下简称溶剂组)和孕酮组,连续2周给药。损伤3个月后进行痛、温觉检测、体感诱发电位(SEP)检测,逆行示踪法计数背根神经节内感觉神经元的数量,脊神经根半薄及超薄切片分别观察髓鞘再生及线粒体形态结构。结果大鼠术后3个月痛觉检测结果:盐水组为(4.69±0.76)N、溶剂组为(474±0.74)N、孕酮组为(1.50±0.33)N,3组比较,孕酮组恢复明显,与盐水组与溶剂组比较,差异均有统计学意义(均P〈0.001)。温觉检测结果:盐水组为(9.50±3.15)s、溶剂组为(10.08±1.88)s、孕酮组为(6.45±1.32)s,孕酮组温觉恢复明显强于盐水组及溶剂组,差异均有统计学意义(均P〈0.05)。体感诱发电位(SEP)检测显示:SEP波幅分别为盐水组(2.25±1.97)μV、溶剂组(2.364-1.68)μV和孕酮组(5.23±3.40)μV,孕酮组SEP波幅高于盐水组及溶剂组,差异均有统计学意义(均P〈0.05);SEP潜伏期分别为盐水组(11.78±0.99)ms、溶剂组(11.63±0.83)ms和孕酮组(11.77±1.07)ms,3组比较差异无统计学意义(P〉0.05)。逆行示踪显示:脊神经节内3组CTB—Alexa555标记阳性神经元数量分别为:盐水组(223±35)个、溶剂组(267±107)个和孕酮组(509±60)个,孕酮组阳性神经元明显多于盐水组与溶剂组,差异均有统计学意义(均P〈0.05)。半薄切片显示:3组C,脊神经根内新生髓鞘的数量分别为(142±64)个、(168±62)个和(102±26)个,3组比较差异无统计学意义(P〉0.05)。脊神经根透射电镜观察显示:盐水组及溶剂组髓鞘板层模糊不清,板层间有间隙及虫蚀样改变,Objective To investigate the effect of progesterone on axonal regeneration and sensory function recovery after brachial plexus injury in rats. Methods An animal model of brachial plexus injury in rats was induced. Twenty-four rats were randomly divided into 3 groups: a saline group, a progesterone vehicle group, and a progesterone group (n = 8 in each group). The rats were administrated continuously for of 2 weeks. Pain, temperature examination, and somatosensory evoked potential (SEP) examination were performed at 3 months after injury. Retrograde tracing method was used to count the number of sensory neurons in dorsal root ganglia. The semi thin and ultrathin sections of spinal nerve roots were used to observe the myelin regeneration and morphology and structure of mitochondria respectively. Results Pain test results at 3 months after procedure in rats : The saline group was 4.69 ± 0. 76 N, the vehicle group was 4.74 ± 0.74 N, and the progesterone group was 1.5± 0. 33 N. Compared with 3 groups, the progesterone group recovered significantly. There was significant difference between the saline group and the vehicle group (all P 〈 0.001 ). Temperature examination results: The saline group was 9.50 ±3.15 s, the solutive group was 10.08± 1.88 s, and the progesterone group was 6.45 ± 1.32 s. The temperature recovery of the progesterone group was stronger than that of the saline group and the vehicle group. There were significant differences (all P 〈 0.05 ). Somatosensory evoked potential (SEP) showed that the SEP amplitude of the saline group was 2.25 ± 1.97 pN, the vehicle group was 2.36 ±1.68 p.V, and the progesterone group was 5.23 ± 3.40 μV. The SEP amplitude of the progesterone group was higher than that of the saline group and the vehicle group. There were significant differences ( all P 〈 0.05 ).
