低浓度钙透析液联合骨化三醇胶丸冲击治疗对维持性血液透析患者继发甲状旁腺功能亢进的影响

摘　　要：目的观察低浓度钙透析液联合骨化三醇胶丸冲击治疗对维持性血液透析患者继发甲状旁腺功能亢进的影响。方法选取绍兴市中医院肾内科血液净化中心2014年2月~2015年2月收治的维持性血液透析患者共70例,按随机数字表法分为观察组和对照组各35例,对照组给予骨化三醇胶丸冲击治疗,观察组给予低浓度钙透析液联合骨化三醇胶丸冲击治疗,随访并记录2组患者全段甲状旁腺激素、Ca^(2+)和P^(5+)指标变化和疗效对比。结果观察组患者治疗后全段甲状旁腺激素、Ca^(2+)和P^(5+)水平分别为[(403.58±103.75)pg/mL、(2.32±0.22)mmol/L、(1.47±0.11)mmol/L)],均优于对照组[(618.93±139.48)、(1.95±0.20)、(1.85±0.19)](P<0.05)。结论低浓度钙透析液联合骨化三醇胶丸冲击治疗显著改善维持性血液透析继发甲状旁腺功能亢进患者的全段甲状旁腺素、Ca^(2+)和P^(5+)水平,无明显副反应。Objective To observe the impact of low calcium dialysate combined with calcitriol capsules pulse therapy on secondary hyperparathyroidism of maintenance hemodialysis patients. Methods Seventy patients undergoing maintenance hemodialysis from February 2014 to February 2015 were randomly divided into observation group and control group with 35 cases in each group. The control group were given calcitriol capsule pulse therapy,observation group were given low calcium dialysate combined with calcitriol capsules. The intact parathyroid hormone,Ca^（2＋）,P^（5＋）indexes were followed up and the efficacy was recorded and compared between two groups of patients. Results The intact parathyroid hormonee,Ca^（2＋）,P^（5＋） indexes in observation group were [（ 403. 58 ± 103. 75） pg / mL,（ 2. 32 ± 0. 22） mmol / L,（ 1. 47 ± 0. 11） mmol / L） ],which improved significantly compared with those in cntrol group[（ 618. 93 ± 139. 48）,（ 1. 95 ± 0. 20）,（ 1. 85 ± 0. 19） ]（ P〈0. 05）. Conclusion Low calcium dialysate combined with calcitriol capsules pulse therapy improves the levels of intact parathyroid hormone,Ca^（2＋）,P^（5＋） in secondary hyperparathyroidism of maintenance hemodialysis patients with no significant side effects.
