中药外敷结合锁定钢板治疗Ⅲ型胫骨平台骨折临床研究

摘　　要：目的:观察中药外敷结合锁定钢板治疗Ⅲ型胫骨平台骨折的临床疗效。方法:选取本院2010年10月—2013年12月诊治的Ⅲ型胫骨平台骨折患者142例,随机分为对照组和观察组各71例,对照组采用锁定钢板治疗,观察组采用中药外敷结合锁定钢板治疗,两组均治疗7 d,比较两组患者的临床疗效、术后恢复情况、膝关节活动度的改变情况、不良事件情况。结果:两组患者胫骨平台内翻角、胫骨平台后倾角均显著增加。观察组患者胫骨平台内翻角、胫骨平台后倾角均明显大于对照组(P<0.05);观察组患者疼痛程度明显好于对照组,观察组患者VAS评分、不良事件发生率均明显低于对照组,观察组患者治疗优良率、Rasmussen评分均明显优于对照组(P<0.05);观察组患者下床活动时间、骨痂形成时间、部分负重时间、完全负重时间均明显早于对照组,观察组患者术后住院时间、骨折愈合时间均明显少于对照组,差异均有统计学意义(P<0.05)。结论:中药洗剂外敷结合锁定钢板是治疗Ⅲ型胫骨平台骨折的有效方法,可明显减轻患者的疼痛并改善患者的膝关节功能,缩短患者的术后恢复时间,不良事件少且安全性高。Objective： To observe the clinical curative effect of external application of Chinese herbs in combination with treatment of locking plate on type III tibial plateau fractures. Methods： One hundred and forty-two patients with type III tibial plateau fractures who were treated in our hospital between Oct. of 2010 and Dec. of 2013 were randomly divided into the control group and the observation group,with 71 cases in each group. The ones in the control group were given treatment of locking plate,while the ones in the observation group were added external application of Chinese herbs based on the treatment of the control group. Then the patients＇ pain degree was evaluated by VAS; their knee joint function was evaluated by Rasmussen score; the two groups＇ clinical curative effect,postoperative recovery,the change of knee joint activity and the incidence of adverse reactions were compared.Results： The two groups＇ tibial plateau inverted angle and inclination angle increased markedly. The tibial plateau inverted angle and inclination angle of the observation group were significantly larger than those of the control group. The pain degree improvement of the observation group was significantly better than that of the control group. The VAS score and the incidence rate of adverse reactions of the observation group were significantly lower than those of the control group. The curing excellent rate and Rasmussen score of the observation group were significantly better than those of the control group. The time for getting out of bed,the time for callus formation,the partial weight-bearing time and full weight-bearing time of the observation group were all significantly earlier than those of the control group. The postoperative hospital stay and fracture healing time of the observation group were significantly less those of than the control group.
