补气养血汤对肾性贫血治疗的时机及靶目标的干预作用

摘　　要：目的:探讨应用补气养血汤配合常规量重组人促红细胞生成素(r Hu EPO)对肾性贫血患者的治疗时机及靶目标的干预作用。方法:将90例肾性贫血患者随机分为4组。A组:补气养血汤同时配合常规量r Hu EPO治疗;B组:补气养血汤择期配合常规量r Hu EPO治疗;C组:单纯应用常规量r Hu EPO治疗;D组:单纯应用补气养血汤治疗。比较A组与其他3组治疗2W、4W、6W的血红蛋白(Hb)、红细胞比容(Hct)、达到靶目标所需的时间,及各组治疗前与治疗后(治疗6W或达到靶目标时)血肌酐(Scr)、血尿素氮(BUN)变化。结果:治疗2W,4W时,A组Hb、Hct升高较B、C、D组明显(P<0.05);A组在治疗4W时达到靶目标值,B、C、D组在第6W达到靶目标值;A组在治疗后4W与B、C、D组在治疗后6W的上述指标的比较,差异无统计学(P>0.05)。4组的血肌酐与尿素氮在治疗前后的差异比较无统计学意义(P>0.05)。结论:补气养血汤同时配合常规量r Hu EPO治疗肾性贫血较其他组治疗达到靶目标的速度快,治疗效果好,且对肾功能的状态有一定的稳定作用。Objective： To explore the intervention effect of buqiyangxuetang combined with regular dose of recombinant human erythropoietin（ r Hu EPO） on the time and target of the treatment of patients with renal anemia.Methods： 90 patients with renal anemia were randomly divided into four groups. Group A were treated with buqiyangxuetang simultaneously coupled with conventional dose of r Hu EPO. Group B were treated with buqiyangxuetang selectively combined with regular dose of r Hu EPO. Group C were treated with regular dose of r Hu EPO alone.Group D were treated with buqiyangxuetang alone. The hemoglobin（ Hb）,hematocrit（ Hct）,and the time needed for the target of group A were respectively compared with the other three group after 2 W、4 W、6 W treatment. And serum creatinine（ Scr） 、blood urea nitrogen（ BUN） in each group after treatment（ 6w treatment or reaching the time of target） were compared to the past. Results： After 2 w,4 w treatment,the level of Hb,HCT in group A were elevated significantly than the group B,C,D（ P〈0. 05）. Group A achieved the target after 4W treatment.The group B、C、D reached the target after 6 W treatment. The level of aforesaid indicators in group A after 4w treatment were no statistical and significant difference compared with group B, C, D after 6 w treatment（ P〉0. 05）. Serum creatinine and urea nitrogen in four groups after treatment was no statistically significant difference than before（ P〉0. 05）. Conclusion： The speed of reaching target is faster and curative effect is better in group treated with buqiyangxuetang simultaneously combined with regular dose of r Hu EPO than the other group,accompanying with stable renal function.
