小剂量卡前列甲酯栓在早孕未产妇人工流产术前的应用

摘　　要：目的:探索早孕未产妇在人工流产术前应用卡前列甲酯栓适宜剂量,达到软化宫颈又能减少不良反应的发生。方法:将300例要求终止妊娠的健康早孕未产妇分为3组,观察组1在术前1h阴道后穹窿放置卡前列甲酯栓0.5mg,观察组2在术前1h阴道后穹窿放置卡前列甲酯栓1mg,对照组未作任何处理情况下行人工流产术,观察患者宫颈软化扩张情况、手术时间、手术出血量、术后流血时间、手术前后血压变化、不良反应和人流综合征的发生。结果:观察组1、组2与对照组比较,宫颈软化扩张情况好、手术时间缩短、术后阴道出血天数减少,未发生人流综合征,差异具有显著性(P<0.05);观察组1和组2比较,软化扩张情况、手术时间、术后阴道出血天数,无显著性差异(P>0.05),而药物不良反应组1少于组2,有显著性差异,(P<0.05)。结论:术前1h放置0.5mg卡前列甲酯栓,可软化宫颈、手术易于操作、术中出血少、术后阴道流血时间短,药物不良反应少,值得临床推广。Objective： To study pregnancy nullipara by using the suitable dosage of Carboprost before induced abortion operation; and to soften the cervix and reduce the occurrence of adverse reactions. Methods： We divided300 healthy women who will terminate the pregnancy into three groups. Group one placed 0. 5mg of Carboprost in the posterior vaginal fornix one hour before the operation. Group two placed 1mg of Carboprost in the posterior vaginal fornix one hour before the operation. In the control group without any treatment in the operation,we observed the cervical softening expansion situation,operation time,blood loss,postoperative bleeding,operation time,the change of blood pressure,and adverse reactions. Results： Group two has the best cervical softening expansion situation,the shortest operation time and the least postoperative vaginal bleeding days,comparing with group one and the control group. The differences are significant（ P〈0. 05）. Conclusion： The effect is the best while placing1 mg of Carboprost one hour before the operation,which can reduce the injury of the cervical. It is worth of clinical promotion.
