颈动脉超声联合hs-CRP检测在冠状动脉粥样硬化性心脏病诊断中的应用与评价

摘　　要：目的探讨颈动脉超声联合hs-CRP检测在冠状动脉粥样硬化性心脏病(CAD)诊断中的应用价值。方法选择2010年3月-2014年5月我院收治的150例CAD患者为观察组,选择同期150例健康体检者为对照组,比较两组患者颈动脉超声结果、血清hs-CRP水平的差异,分析影响冠脉狭窄的危险因素。结果观察组动态动脉硬化指数(AASI)、Crouse斑块积分、斑块发生率、内膜-中膜厚度(IMT)及血清hs-CRP水平明显高于对照组(P<0.01)。随着CAD患者冠脉狭窄越严重,其AASI、Crouse斑块积分、IMT、hs-CRP水平和斑块发生率呈明显的递增趋势(P<0.05)。结论血清hs-CRP升高与CAD冠脉狭窄存在显著相关性。hs-CRP水平联合颈动脉超声检测对于CAD的临床诊断及冠脉狭窄程度的预测具有重要意义。Objective To study diagnostic value of carotid artery ultrasound combined with hs-CRP detection in diagnosis of coronary atherosclerotic heart disease（CAD）.Methods 150 cases of patients with CAD in our hospital from March 2010-May 2014 were selected as the observation group,and 150 cases of healthy persons at the same period were selected as the control group.The results of carotid artery ultrasound and the levels of serum hs-CRP between the two groups were compared.according to the results of carotid artery ultrasound,the patients with CAD were divided into mild,moderate,severe stenosis group,the risk factors of coronary artery stenosis were analyzed.Results The ambulatory arterial stiffness index（AASI）,Crouse plaque scores,incidence of the plaque,intima-media thickness（IMT）and levels of serum hs-CRP of the observation group were significantly higher than those of the control group（P〈0.01）.With the more serious the coronary artery stenosis in patients with CAD,its AASI,Crouse plaque integral,IMT,hsCRP levels and incidence of plaque was obvious increasing（P〈0.05）.Conclusion there is a significant correlation between elevated Serum hs-CRP and CAD coronary artery stenosis.The levels of serum hs-CRP combined with carotid artery ultrasound has important significance of clinical diagnosis of CAD and prediction of the degreed of coronary artery stenosis.
