天楼解毒消肿散外用治疗糖尿病足100例临床观察

摘　　要：目的观察天楼解毒消肿散外用治疗糖尿病足的临床疗效及安全性。方法将150例糖尿病足患者随机分为治疗组100例和对照组50例。两组患者均给予降糖、降压、降脂、抗菌等基础治疗,治疗组加用天楼解毒消肿散适量直接涂抹于溃疡处,对照组加用如意金黄散适量涂抹患处或用凡士林调和后摊于纱布上贴患处。两组均每日1次,治疗4周。比较两组治疗前后临床症状积分、溃疡面积,治疗后评价中医证候疗效。结果治疗组中医证候疗效总有效率为91%,对照组为72%,治疗组明显优于对照组(P<0.05)。两组治疗后病灶范围、肿势、发热、疼痛、成脓性质、腐肉生成积分及临床症状总积分均较本组治疗前下降,溃疡面积亦明显下降(P<0.05),且治疗组优于对照组(P<0.05)。结论天楼解毒消肿散外用能有效改善糖尿病足患者临床症状,缩小溃疡面积,安全性好。Objective To observe the clinical efficacy and safety of Tianlou Detoxification Detumescence Powder（ 天楼解毒消肿散） on diabetic foot. Methods One hundred and fifty cases of diabetic foot patients were randomly divided into the treatment group with 100 cases and the control group with 50 cases. Two groups of patients were given basic treatment,the treatment group was treated with Tianlou Detoxification Detumescence Powder directly applied to the ulcer,the control group was treated with Ruyi Jinhuang Powder（ 如意金黄散） smeared onto the affected part or with Ruyi Jinhuang Powder mixed with Vaseline spread onto the gauze and attached to the ulcer. Both groups were treated 1 time daily for 4 weeks. The clinical symptom score and the ulcer area of the two groups before and after treatment and the TCM syndrome curative effect after treatment were compared. Results The Chinese medicine syndrome total effective rate was 91% and 72% in the treatment group and control group respectively,the treatment group was significantly superior to the control group（ P〈0. 05）. After treatment,the scope of lesions,swelling potential,fever,pain,the nature of sepsis,carrion generation,the total symptom score and ulcer area of the two groups were significantly decreased（ P〈0. 51）,and the treatment group was significantly superior to the control group（ P〈0. 05）. Conclusion Tianlou Detoxification Detumescence Powder can effectively improve the clinical symptoms of patients with diabetic foot,which can reduce the ulcer area and has good security.
