老年高血压患者血同型半胱氨酸与血压昼夜节律、动脉硬化的关系

摘　　要：目的探讨老年高血压患者血同型半胱氨酸(Hcy)与血压昼夜节律及动脉硬化的关系。方法将133例老年高血压患者按照血Hcy水平分为非H型高血压组63例,H型高血压组70例,分别测定2组患者的体质量指数、血糖、血脂水平、24 h动态血压以及脉搏波传导速度、踝臂指数。结果 H型高血压组和非H型高血压组的日间平均收缩压、日间平均舒张压均无显著差异(P>0.05);H型高血压组的夜间平均收缩压及夜间平均舒张压均明显高于非H型高血压组(P<0.05);H型高血压组夜间收缩压下降率、夜间舒张压下降率均明显低于非H型高血压组(P<0.05);H型高血压组患者血压昼夜节律异常发生率明显高于非H型高血压组(P<0.05);H型高血压组的脉搏波传导速度明显高于非H型高血压组(P<0.01)。结论老年高血压病患者血Hcy水平升高与血压昼夜节律紊乱、动脉硬化密切相关。Objective To investigate the relationship between homocysteine（ Hcy） and circadian rhythm of blood pressure,pulse wave velocity in elderly patients with hypertension. Methods A total of 133 elderly patients with hypertension were divided into non-H-type hypertension group（ n = 63） and H-type hypertension group（ n = 70） according to Hcy levels. The body mass index,levels of blood glucose,blood lipid level,24 h ambulatory blood pressure,pulse wave velocity（ ba PWV） and ankle-brachial index（ ABI） of all patients were determined. Results No significant differences were found between two groups in day mean systolic blood and day mean diastolic blood pressure（ P〉0. 05）; But night mean systolic blood pressure and night mean diastolic blood pressure were higher in the H-type hypertension group（ P〈0. 05）; The decrease rate of night systolic blood pressure and night diastolic blood pressure in the H-type hypertension group was lower than that in the non-H-type hypertension group（ P〈0. 05）. The abnormalities incidence of circadian rhythm of blood pressure in the H-type hypertension group was higher than that in the non-H-type hypertension group（ P〈0. 05）. The ba PWV was higher in the H-Type hypertension group than that in the non-H-type hypertension group（ P〈0. 01）. Conclusion The rise of Hcy level is closely related with circadian rhythm of blood pressure and arteriosclerosis of elderly patients with hypertension.
