摘  要:目的探讨急性心肌梗死行急诊冠脉介入手术术中并发电风暴的抢救与护理。方法回顾性分析9例急性心肌梗死行急诊冠脉介入手术中发生电风暴患者的临床资料,均予急救等处理,并全面加强护理,观察抢救后预后。结果 9例患者发生电风暴时间平均为(23.1±1.8)h,其中2例在6 h内出现,3例在术后6~12 h出现,2例12~24 h出现,1例24~48 h出现,1例48 h以后出现。电风暴持续时间平均(104.2±81.4)min;电击次数平均(9.8±5.7)次。经过一系列抢救护理后,24 h成功率为77.78%,2周内存活率为55.56%,1年内存活率为44.44%,24 h内死亡因频繁室颤,2周内死亡因心力衰竭,1年内死亡因术后并发症如心脏骤停等。结论急性心肌梗死患者行急诊冠脉介入手术术中并发电风暴经全面抢救和护理后,预后较满意。Objective To investigate rescue and nursing of electrical storm patients with acute myocardial infarction during coronary artery intervention Methods The clinical data of 9 acute myocardial infarction patients with electrical storm during percutaneous coronary intervention was retrospectively analyzed,and all the patients were given first aid treatment,and rescue prognosis was observed Results The average time of electrical storm in 9 patients was( 23. 1 ± 1. 8) h,2 cases occurred within 6 h,3 cases in postoperative 6 ~ 12 h,2 cases in 12 ~ 24 h,1 case in 24 ~ 48 h,and1 case in 48 h The average duration of the electrical storm was( 104. 2 ± 81. 4) min The mean number of shocks was( 9. 8 ± 5. 7) times After a series of treatments and nursing,success rate of 24 h was 77. 78%,the survival rate was 55. 56% within 2 weeks,1 year survival rate was 44. 44% Death in 24 h was caused by frequent ventricular fibrillation,death in 2 weeks by heart failure,death within1 year by postoperative complications such as cardiac arrest and so on Conclusion Comprehensive rescue and nursing for acute myocardial infarction patients with electrical storm during coronary intervention has achieved satisfactory prognosis
