摘  要:目的:从舌苔的微生态出发,研究食管癌舌苔变化的特征,探讨食管癌患者的舌苔微生态情况,丰富中医学舌诊的内容。方法:通过搜集患者以及正常人的舌苔,观察舌苔菌群的密集度、舌苔细菌的多样性、细菌的比例以及舌苔的p H值。结果:1食管癌黄厚腻苔组和反流性食管炎黄厚腻苔组分别在舌苔菌群密集度和菌群的多样性上与正常人的薄白苔相比均显著增高(P<0.05),但是患病组之间对比无明显差异,在显微镜下观察后发现正常人涂片背景较为清晰干净,而患病组则细菌附着较多且背景杂乱无章。2食管癌黄厚腻苔组、反流性食管炎黄厚腻苔组舌苔的p H值比健康人薄白苔的p H值有所下降,但是下降幅度不大,差异无统计学意义(P>0.05)。3食管癌黄厚腻苔组革兰阳性杆菌较健康对照组、反流性食管炎黄厚腻苔组明显减少(P<0.05);革兰阳性球菌较健康对照组明显减少(P<0.05),较反流性食管炎黄厚腻苔组明显增多(P<0.05);革兰阴性杆菌较健康对照组明显增多(P<0.05),较反流性食管炎黄厚腻苔组明显减少(P<0.05);革兰阴性球菌较健康对照组明显增加(P<0.05),与反流性食管炎黄厚腻苔组相比无明显差异(P>0.05)。结论:舌苔与微生物之间关系密切,不同疾病所示相同舌苔在细菌比例上有差异,舌苔的菌群密集度、舌苔菌群的多样性及舌苔p H值在食管癌和反流性食管炎的形成过程中有着重要意义。Objective: This topic from tongue coating micro ecology,the study of the characteristics of changes of tongue coating,coating on the tongue on patients with esophageal micro ecological condition,to rich the content of the tongue diagnosis in traditional Chinese medicine. Methods: Through collecting patients and normal tongue coating,observed the intensity of tongue coating flora and bacterial diversity of tongue coating,the proportion of the bacteria,and the PH of the coating on the tongue. Results: ①Esophageal cancer yellow thick greasy tongue group and reflux esophageal yellow thick greasy tongue group respectively in tongue coating bacteria concentration of bacteria and group diversity were significantly increased compared with normal people thin and whitish fur( P〈0. 05),but the prevalence between groups comparison showed no significant difference,under the microscope observation found that normal smear background was clear and clean,sick group bacterial attachment and more background clutter. ②Esophageal cancer group p H and esophagitis group tongue p H were declined combined with the healthy person thin and whitish fur,but the decline range was not big,the difference was not statistically significant( P〈0. 05). ③Esophageal cancer yellow thick greasy fur group of gram positive bacilli compared to healthy controls and reflux esophagitis group decreased significantly( P〈0. 05);Gram positive cocci compared with the healthy control group decreased significantly( P〈0. 05) and esophagitis group increased significantly( P〈0. 05); Gram negative bacilli compared with the healthy control group increased significantly( P〈0. 05),compared with esophagitis group decreased significantly( P〈0. 05); Gram negative cocci compared with the healthy control group increased significantly( P〈0. 05),compared with esophagitis group no significant difference( P〈0. 05).
