天津市监测点与非监测点医院乙型肝炎疫情报告特征与质量研究

摘　　要：目的了解肝炎监测点与非监测点医院乙型肝炎(乙肝)诊断报告特征及质量,初步探讨设立监测点效果。方法采用描述流行病学方法,对2005-2013年《中国疾病预防控制信息系统》终审报告的现住址天津的乙肝病例报告、分类、实验室确诊、订正等情况按是否监测点进行统计分析和比较。结果监测点医院终审有效报告病例数(34 910例)占全市乙肝报告总数的74.76%,高于非监测点(26 100例)的25.24%;监测点分类率、实验室诊断率均高于设立前,也高于非监测点,删除订正率低于非监测点(P<0.05)。结论设立肝炎监测点能够高效推动天津乙肝疫情诊断和报告质量的快速提高。Objective To investigate the characteristics and quality of diagnosis report on hepatitis B in surveillance and non surveillance hospitals in Tianjin City, and to preliminarily explore the effectiveness of the establishment of surveillance hospitals. Methods We employed descriptive epidemiological method to statistically analyze the data about hepatitis B cases, classification, laboratory diagnosis and correction in Tianjin City during 2005 - 2013 collected from the final verified reports of China Information System for Disease Control and Prevention, and to compare the data between surveillance hospitals and non- surveillance hospitals. Results Totally, 34,910 cases were finally verified and reported by the surveillance hospitals, accounting for 74.76% of the total number of the HBV cases in Tianjin City, and more than those by the nonsurveillance hospitals （26,100 cases, accounting for 25.24 % ）. The classification rate and laboratory diagnosis rate of HBV in the surveillance hospitals after surveillance establishment were both higher than those before and than those of the non surveillance hospitals （P 〈 0.05）. The deletion and correction rate of the surveillance hospitals was lower than that of the non - surveillance hospitals （ P 〈 0.05 ）. Conclusion Establishing hepatitis surveillance hospitals can rapidly and effectively improve the diagnosis and reporting quality of hepatitis B in Tianjin City.
