乙型肝炎病毒感染者血清HBV-LP检测意义及其与PreS1抗原、HBV DNA之间关系的研究

摘　　要：目的初步探讨HBV感染者血清中的乙肝大蛋白（HBV—LP）检测的临床意义，并对其与PreS1抗原、HBVDNA之间的关系进行分析。方法采用酶联免疫吸附试验（ELISA）方法检测192例HBV感染者血清中的HBV—LP，PreS1抗原及HBV分子标志物（HBV M），并用荧光定量PCR方法检测HBV DNA。结果192例HBV感染者血清HBV—LP．PreS1抗原、HBVDNA及HPeAg阳性率分别为67．18％、54．17％、52．60％和9．38％，HBV—LP阳性率高于PreS1抗原阳性率（χ^2=6．82，P〈0．05）、HBVDNA检测阳性率（χ^2=8．50，P〈0．05）及HBeAg阳性率（χ^2=135．80，P〈0．05）；HBV—LP含量（A值）与HBVDNA拷贝数的对数值呈正相关关系（r=0．798，P〈0．05）。结论血清中HBV—LP的含量与HBVDNA的拷贝数相关性较好，HBV—LP能准确的反映乙肝病毒复制情况，是HBeAg、PreS1抗原的重要补充。Objective To preliminarily explore the clinical value of serum hepatitis B virus large surface protein( HBV- LP)detection,and to analyze the relationship between HBV- LP and Pre S1 antigen,HBV DNA copies. Methods Enzyme linked immuno- sorbent assay( ELISA) was used to detect HBV- LP,Pre S1- Ag and molecular markers of HBV in 192 serum samples of HBV patients,and HBV DNA was quantitatively determined by real- time polymerase chain reaction( PCR). Results Among 192 serum samples of HPV patients,the positive rates of HBV- LP,Pre S1- Ag,HBV DNA,and HBe Ag were 67. 18%,54. 17%,52. 60% and 9. 38% respectively. The positive rate of HBV- LP was higher than those of Pre S1- Ag( χ~2= 6. 82,P <0. 05),HBV DNA( χ~2= 8. 50,P < 0. 05),and HBe Ag( χ~2= 135. 80,P < 0. 05). The content of HBV- LP( A value) was positively correlated with the logarithm value of the number of HBV DNA copies( r = 0. 798,P < 0. 05). Conclusions There was a good correlation between the content of serum HBV- LP and the number of HBV DNA copies. Serum HBV- LP can accurately reflect the state of HBV DNA reproduction,and it is an important complementarity marker to traditional HBe Ag and Pre S1- Ag.
