摘 要:目的调查天津市离退休老年人血同型半胱氨酸(homocysteine,Hcy)水平及高Hcy血症(Hcy≥15μmol/L)患病的影响因素。方法从2014年6月~8月天津三所三甲大型综合医院查体离退休1247例老年人中,筛选出资料完整者877例,根据血清Hcy水平,分为高Hcy组(≥15μmol/L)和正常Hcy组(〈15μmol/L),应用多元Logistic回归分析高Hcy血症的相关因素。结果 877例离退休老年人高Hcy血症者占53.48%,平均水平为(21.50±7.86)μmol/L;高Hcy组男性比例,年龄,体质指数及尿酸水平均明显高于正常Hcy组(P〈0.005);高Hcy组痛风及眼底动脉硬化比例较正常Hcy组偏高(P〈0.05);两组间冠心病,高血压,糖尿病,陈旧脑梗死等各疾病比例差异无显著性(P〉0.05)。Pearson相关性分析结果显示:血Hcy水平与年龄(rp=0.324,p=0.000),尿酸水平(rp=0.283,p=0.001)成正相关。多元logistic回归分析显示年龄,性别和血尿酸水平和眼底动脉硬化是影响离退休老年人血高Hcy血症的危险因素。结论年龄,性别,血尿酸水平和眼底动脉硬化是影响离退休老年人血清Hcy水平的危险因素。Objective To investigate the influential factors of serum Hcy(homocysteine) level of retired-elderly people in Tianjin. Methods Recruit a total of 877 participants, who were from 1247 retired people of three large tiertiary hospitals in Tianjin from June to August in 2014. They were divided into two groups by the level of serum Hcy, that was high level of serum Hcy(≥15μmol/L) and normal level of serum Hcy(〈15μmol/L). Explore the influential factors of serum Hcy by logistic regression analysis. Results The high Hcy proportion of retired- elderly people accounted for 53.48%, and mean Hcy concentration was(21.50±7.86)μmol/L. Age, BMI and uretic acid on the group of high Hcy concentrations were higher compared with normal Hcy group(P〈0.005). The male patients had the higher concentrations of serum Hcy than that of the female patients(P〈0.005). There were no statistical significance in patients with hypertension, type 2 diabetes mellitus, old cerebral infarction and other diseases between two groups. Pearson correlation analysis showed that the level of serum Hcy had a positive corralation with age and uric acid, and correlation coefficient was 0.324(p=0.000) and 0.283(p=0.001) respectively. Multiple logistic regression analysis showed that age, sex, uric acid and ocular fundus arteriosclerosis were risk factors for elevation of serum Hcy in retired-elderly people. Conclusions Age, sex, serum uric acid and ocular fundus arteriosclerosis were risk factors for serum Hcy levels in retired-elderly people.
