摘 要:目的观察应用MIPO(minimally invasive plate oseoynthesis)技术治疗Pilon骨折的临床效果,探讨在高能量Pilon骨折应用MIPO技术治疗的临床应用价值。方法回顾分析2008年8月~2012年8月间38例在我院应用MIPO技术治疗的高能量Pilon骨折患者为MIPO技术组。入组患者平均年龄39.2岁,按照Ruedi-Allgower骨折分型,其中Ⅱ型28例,Ⅲ型10例;对照组采用切开复位内固定术,共纳入42例患者,Ⅱ型骨折25例,Ⅲ型骨折17例。采用踝关节Olreud-Molander功能评分对踝关节功能进行评价。对两组的手术前后X线资料和随访结果进行数据分析。结果两组病例均获得随访,平均随访时间为1.9年(1~3.2年)。两组患者Olreud-Molander踝关节评分差异具有统计学意义(P〈0.05)。结论结合完善的术前计划、恰当的手术时机,应用MIPO技术治疗高能量Pilon骨折能够取得良好的效果。Objective To investigate the clinical outcome of MIPO(minimally invasive plate oseoynthesis) technique in the treatment of Pilon fracture and discuss the clinical application of MIPO technique in the treatment of high energy induced Pilon fractures. Methods A total of 38 patients with high-energy Pilon fractures treated by MIPO technology in our department from August 2008 to August 2012 were retrospective analyzed. Average age was 39.2 years old. The Pilon fractures were classified according to the classification of Ruedi-Allgower type, type Ⅱ 28 cases, typeⅢ 10 cases.The control group treated by open reduction and internal fixation 42 cases, included type Ⅱ 25 cases, type Ⅲ 17 cases.Olreud-Molander ankle function score was used to evaluated the function after treatment. The clinical effect was assessed according to the X-ray before and after treatment and follow-up results. Results The patients were followed up between 1-3.2 years, average 1.9 years. The Olreud-Molander ankle function score of two groups had statistically significant differences(P〈0.05). Conclusion The preoperative planning, correct choice of operation time, applied of MIPO technique in the treatment of high energy Pilon fracture can achieve good curative effect.
