摘  要:目的探讨早期血乳酸清除率在判断新生儿重度窒息预后中的作用。方法回顾性分析2009年12月~2014年12月台州恩泽医疗集团路桥医院和台州医院新生儿科重度窒息患儿77例,根据早期血乳酸清除率划分为高乳酸清除率组45例和低乳酸清除率组32例,对两组0 h和6 h血乳酸水平,6 h乳酸清除率和病死率进行比较,并对6 h乳酸清除率和病死率进行相关性分析。结果两组0 h血乳酸清除率比较,高乳酸清除率组为(6.17±1.03)mmol/L,低乳酸清除率组为(6.57±1.06)mmol/L,差异无统计学意义(t=1.65,P〉0.05);两组6 h乳酸水平比较,高乳酸清除率组为(5.07±0.82)mmol/L,低乳酸清除率组为(6.24±1.03)mmol/L,差异有统计学意义(t=6.10,P〈0.01);两组6 h乳酸率比较,高乳酸清除率组病死率为(17.57±2.49)%,低乳酸清除率组为(4.49±1.36)%,差异有高度统计学意义(t=29.54,P〈0.01);两组病死率比较,高乳酸清除率组病死率为4.44%,低乳酸清除率组为28.13%,差异有高度统计学意义(χ~2=8.55,P〈0.01);新生儿重度窒息6 h乳酸清除率和病死率呈正相关(r=0.88,P〈0.01)。结论 6 h乳酸清除率可用于判断新生儿重度窒息预后。Objective To explore the effect of early blood lactose clearance rate in the judgment of the prognosis of neonatal severe asphyxia. Methods 77 children patients with severe asphyxia who were admitted to the Neonatal Department in Taizhou Enze Medical Group Luqiao Hospital and Taizhou Hospital from December 2009 to December2014 were retrospectively analyzed. They were assigned to the group of high lactose clearance rate with 45 patients and the group of low lactose clearance rate with 32 patients according to the early blood lactose clearance rate. 0 h and 6 h blood lactose level, 6 h lactose clearance rate and mortality rate were compared between the two groups, and correlation analysis was carried out for 6 h lactose clearance rate and mortality rate. Results In the comparison of 0 h blood lactose clearance rate between the two groups, the group of high lactose clearance rate was(6.17 ±1.03) mmol/L, and the group of low lactose clearance rate was(6.57±1.06) mmol/L. The difference was not significant(t=1.65, P〉0.05); in the comparison of 6h lactose level between the two groups, the group of high lactose clearance rate was(5.07 ±0.82) mmol/L,and the group of low lactose clearance rate was(6.24±1.03) mmol/L. The difference was significant(t=6.10, P〈0.01); in the comparison of 6h lactose clearance rate between the two groups, the lactose clearance rate in the group of high lactose clearance rate was(17.57±2.49)%, and the rate in the group of low lactose clearance rate was(4.49 ±1.36)%. The difference was significant(t=29.54, P〈0.01); in the comparison of mortality rate between the two groups, the mortality rate in the group of high lactose clearance rate was 4.44%, and the mortality rate in the group of low lactose clearance rate was 28.13%, the difference was significant( χ~2=8.55, P〈0.01); 6 h lactose clearance rate in the newborns with neonatal severe asphyxia was positively correlated to the mortality rate(r=0.88, P〈0.01).
