摘  要:目的 探讨实验室诊断人布鲁菌病(布病)快速、准确及简便的检测技术。方法采用试管凝集试验(SAT)、间接酶联免疫吸附试验(IELISA)、荧光偏振试验(FPA)和血培养方法对95例确诊(确诊病例组)及42例疑似布病患者(疑似病例组)和同期56例健康体检者(健康对照组)进行布鲁菌实验室检测。分析上述方法在布鲁菌病诊断中的价值。结果SAT、IELISA、FPA及血培养方法检测人血清布鲁菌在确诊病例组中的阳性率(100.0%、98.9%、96.8%、17.9%)均高于疑似病例组(0、7.1%、4.8%、0,P均〈0.01)和健康组(1.8%、0、3.6%、未做血培养,P均〈0.01)。确诊病例组中,急、亚急性及慢性病例间比较,IELISA、FPA检测布鲁菌抗体及血培养阳性率间差异无统计学意义(P均〉0.05)。IELISA试验的敏感性(98.9%)、特异性(100.0%)和Yonden指数(0.989)最高。FPA敏感性和特异性分别为96.8%和96.4%。结论采用FPA对布鲁菌抗体进行初筛,IELISA进行确诊相结合的实验室检测方法,能更简便、迅速地对布鲁菌病进行诊断。Objective In order to develop a rapid, accurate and convenient practicable technique for diagnosis of brucellosis. Methods Brucella antibody or bacteria was detected by standard agglutination test (SAT), Indirect enzyme-linked immunosorbent assay(IELISA), fluorescence polarization assay (FPA) and blood culture tests in 95 confirmed and 42 suspected patients and 56 healthy controls during the same period. The diagnostic value of SAT, IELISA, FPA and blood culture tests were analyzed. Results Positive rates of confirmed patients (100.0%, 98.9%, 96.8% and 17.9%) by SAT, IELISA, FPA and blood culture tests were significantly higher than those of suspected patients (0, 7.1%, 4.8% and 0, all P 〈 0.01) and those in healthy controls (1.8%, 0, 3.6% and no blood culture, all P 〈 0.01). Positive consistency rates were not different in acute,subacute and chronic patients with IELISA, FPA and blood culture tests (all P 〉 0.05). IELISA showed the highest sensitivity (98.9%), specificity (100.0%) and Yonden index (0.989). The sensitivity and specificity of FPA were 96.8% and 96.4%. Conclusions Brucella antibody is detected conveniently, rapidly, tentatively by FPA, and it is confirmed by IELISA, which is an important tech support in prevention and cure of Brucella disease and is worth of applying extensively.
