摘  要:目的探讨磁共振弥散加权成像(diffusion-weighted imaging,DWI)技术在肝炎纤维化程度评估中的价值,以早期发现肝纤维化,防止肝硬化、肝癌的发生。方法对43例慢性病毒性肝炎患者和10名无肝脏病史、无肝功能异常的受试者行磁共振DWI检查,在弥散敏感系数b值分别取0、500、1000s/mm2的情况下,测定感兴趣区的表观弥散系数(apparent diffusion coefficient,ADC),两周后行肝脏穿刺活检,病理切片进行常规HE染色,采用Knodell组织学活动度指数(histological activity index,HAI)积分系统进行慢性肝炎纤维化程度分期,采用SPSS 170统计学软件进行数据分析,将慢性肝炎患者不同肝叶的ADC值与正常对照组进行对比,并将不同肝叶的ADC值与HAI评分及肝纤维化分期进行相关性统计分析。结果慢性肝炎组ADC值低于正常对照组(P<005),左外叶、左内叶及右前叶的ADC值亦低于正常对照组的相应ADC值,各叶ADC值与HAI及肝纤维化呈正相关。结论 DWI技术可用于评价慢性病毒性肝炎的纤维化程度。ObjectiveTo investigate the value of diffusion-weighted imaging(DWI) in the assessmentof liver fibrosis,so as to detect liver fibrosis earlierly and prevent the development of hepatic cirrhosis andcarcinoma.MethodsTotally 43 hepatitis patients and 10 healthy volunteers were included in this study. All ofthem underwent DWI on a 3 0T magnetic resonance imaging system with b value 0, 500, 1000 s/ mm2,respectively. Liver biopsy was performed 2 weeks after DWI,normally hematoxylin and eosin(HE) staining.Biopsy specimens were scored for chronic hepatitis fibrosis according to the Knodell histological activity index(HAI). Statistical Package for the Social Sciences( SPSS) 17 0 was used for the analysis. The apparentdiffusion coefficient( ADC) values of liver segments in patients and controls were compared. Then therelationship between fibrosis and HAI in patients with respect to liver segments was also statistically analyzed.ResultsThe ADC values of patients were lower than the normal controls. There were positive relationshipsbetween ADC and HAI,ADC and fibrosis.ConclusionsDWI can be used to evaluate the stages of liver fibrosisin hepatitis patients.
