摘  要:目的:探讨腮腺良性肿瘤的磁共振成像(MRI)表现及其诊断价值。方法:回顾性分析22例经手术或穿刺病理证实的腮腺良性肿瘤患者的MRI资料,并与手术病理结果进行对比分析。结果:22例腮腺良性肿瘤均与正常腮腺组织分界清;11例多形性腺瘤中9例位于腮腺浅叶,2例位于腮腺深叶,在T_1WI上呈等或低信号,T_2WI上呈高信号,增强后明显强化,其中7例呈渐进性强化;8例腺淋巴瘤均位于腮腺浅叶,在T_1WI上高信号,T_2WI上呈等或低信号,增强后轻度不均匀强化;2例基底细胞腺瘤,1例位于腮腺深叶,1例位于腮腺浅叶,T_1WI上呈等信号,T_2WI上呈高信号,增强后明显强化;1例浆细胞型肌上皮瘤,无特征性表现。结论:腮腺良性肿瘤MRI表现有一定特征性,故MRI在腮腺良性肿瘤的诊断和鉴别诊断中有较高的临床应用价值。Objective: To evaluate the performance and diagnosis value of MRI to benign parotid tumors. Methods: The MRI characteristics of 22 patients with surgery and pathology proved parotid tumors were reviewed retrospectively, and contrasted with the surgery and pathology findings. Results: Twenty two cases of benign parotid tumors showed clear interface with the normal parotid tissue. The location of the 11 pleomorphic adenomas were superficial lobe(n=9) and deep lobe(n=2), showing iso-/hypo-intensity on T1 WI, hyper-intensity on T2 WI, markedly enhancement after contrast administration, of which 7 cases showed progressive enhancement. The location of the 8 Warthin tumors were superficial lobe, showing hyper-intensity on T1 WI, iso-/hypo-intensity on T2 WI, slight enhancement after contrast administration. The location of the 2 basal cell adenoma were superficial lobe(n=1) and deep lobe(n=1), showing iso-intensity on T1 WI, hyper-intensity on T2 WI, markedly enhancement after contrast administration. 1 case of plasma cell myoepithelioma have no characteristic manifestations. Conclusion: There were certain MRI characteristics in different common benign parotid tumors, MRI showed important clinical values in the diagnosis and differential diagnosis.
