摘  要:目的比较局部麻醉和全身麻醉对肺癌患者血清IL-4、IFN-γ和IL-18水平的影响。方法选取笔者所在医院收治的肺癌患者80例,根据不同的麻醉方法随机分为局部麻醉组和全身麻醉组,每组40例。局部麻醉组行硬膜外麻醉,全身麻醉组行静吸复合全麻,分别使用酶联免疫吸附试验(ELISA)测定两组患者在麻醉诱导前(T0)、手术切皮(T1)和手术开始后1 h(T2)的血清细胞因子白细胞介素-4(IL-4)、干扰素-γ(IFN-γ)和白细胞介素-18(IL-18)的水平。结果局部麻醉组患者在T1和T2时血清中IL-4、IFN-γ和IL-18水平明显高于T0时,差异具有统计学意义(P<0.05);全身麻醉组患者在T2时血清中IL-4、IFN-γ和IL-18水平与T0时比较,差异没有统计学意义(P>0.05)。T1、T2时,局部麻醉组患者血清中IL-4、IFN-γ和IL-18水平均明显高于全身麻醉组,差异具有统计学意义(P<0.05)。结论硬膜外麻醉能够明显提高肺癌患者血清中IL-4、IFN-γ和IL-18水平,有利于保护患者的免疫系统,提高肿瘤患者的抗炎能力。Objective To compare the effects of local and general anesthesia on the IL-4,IFN-γ and IL-18 levels of lung concer patients. Methods The 80 patients with lung cancer admitted in author's hospital were selected as the research objects and divided into the local anesthesia group and general anesthesia group,for each group,40 cases,according to the different methods of anesthesia. Local anesthesia group was given the epidural anesthesia and general anesthesia group was given the inhalation anesthesia combined with the general anesthesia.Blood samples were collected before induction of anesthesia(T0),skin incision(T1) and one hour after skin incision(T2). And IL-4,IFN-γ and IL-18 levels were measured using enzyme-linked immunosorbent assay(ELISA).Results IL-4,IFN-γ and IL-18 levels at T1 and T2of local anesthesia group were significantly higher than at T0 the difference was significant statistically significant(P〈0.05). No significant changes were observed in general anesthesia group's IL-4,IFN-γ and IL-18 levels at T2 and at T0(P〉0.05). The levels of IL-4,IFN-γ and IL-18 at T1and T2 of regional anesthesia group were significantly higher than these at T1 and T2in general anesthesia group the difference was significant statistically significant(P〈0.05). Conclusion Epidural anesthesia can significantly increase the levels of IL-4,IFN-γ and IL-18 of patients with lung cancer,which is conducive to protect the patients' immune system,and improve the anti-inflammatory ability in caner patients.
