摘 要:目的探讨肝郁影响围绝经期非器质性失眠的可能机制。方法收集围绝经期非器质性失眠妇女102例,参照《证素辨证学》对围绝经期妇女进行肝郁证素积分和肝郁分级,采用匹兹堡睡眠质量指数(PSQI)量表评价失眠的严重程度,测定血清中白细胞介素1β(IL-1β)、肿瘤坏死因子-α(TNF-α)卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)水平。分析肝郁证素积分、肝郁分级、PSQI量表评分、FSH、LH、E2与IL-1β、TNF-α的相关性。结果 IL-1β、TNF-α均与FSH、LH、E2无明显相关性(P>0.05);IL-1β、TNF-α均与肝郁证素积分呈正相关(P<0.05);IL-1β与TNF-α呈正相关(P<0.001);PSQI量表评分与肝郁证素积分呈正相关(P<0.001)。肝郁分级越高,IL-1β、TNF-α水平越高,且两者在不同肝郁分级间的差异有统计学意义(P=0.003,P=0.019)。与肝郁0级比较,2、3级IL-1β,3级TNF-α显著升高(P<0.01);与肝郁1级比较,3级IL-1β、TNF-α均显著升高(P<0.05)。结论细胞因子IL-1β、TNF-α可能参与肝郁所致围绝经期非器质性失眠女性的发病发展。Objective To investigate the possible mechanism of effect of liver depression on perimenopausal women with nonorganic insomnia. Methods One hundred and two cases of perimenopausal women with nonorganic insomnia were collected,according to the "syndrome element differentiation",the liver depression syndrome score and liver depression,graded the chosen perimenopausal women,Pittsburgh Sleep Quality Index( PSQI) scale was used to evaluate the severity of insomnia,the serum interleukin-1β( IL-1β),tumor necrosis factor-α( TNF-α) follicle stimulating hormone( FSH),progesterone( LH),estradiol( E2) level were tested. Correlation analysis of liver depression syndrome element score,liver depression grade,PSQI score,FSH,LH,E2 and IL-1β TNF-α were taken.Results There is no significant correlation between IL-1β,TNF-α and FSH,LH,E2( P 〉 0. 05); IL-1β,TNF-αare positively correlated with liver depression syndrome score( P 〈 0. 05); IL-1β is positively correlated with TNF-α( P 〈 0. 001); The score of PSQI is positively correlated with liver depression syndrome element score( P 〈 0. 001).The higher the grade of liver depression,IL-1β and TNF-α level are higher,and the difference of them in different grades of liver depression have statistical significance( P = 0. 003,P = 0. 019). Compared with grade 0 of liver depression,IL-1β of grade 2 and grade 3,TNF-α of grade 3 increased significantly( P 〈 0. 01); Compared with grade1 of liver depression,both IL-1β and TNF-α of grade 3 increased significantly( P 〈 0. 01). Conclusion Cytokine IL-1β and TNF-α may be involved in the occurrence and the development of nonorganic insomnia of perimenopausal women caused by liver depression.
