肝硬化腹水并发自发性细菌性腹膜炎的危险因素

摘　　要：目的探讨肝硬化腹水并发自发性细菌性腹膜炎的危险因素。方法选取汉中市中心医院2014年1月-2015年12月收治的50例肝硬化腹水并发自发性细菌性腹膜炎患者作为观察组，选取同期肝硬化腹水未发生细菌性腹膜炎患者50例作为对照组。对两组患者性别、年龄、血清白蛋白、血清总胆红素、血小板、凝血酶原时间国际标准化比值、腹水持续时间、合并症、肝硬化形成原因进行单因素分析，对差异有统计学意义的单因素进行Logistic回归分析。结果通过量因素分析，两组患者饮酒、血清白蛋白、血清总胆红素比较差异有统计学意义（P〈0．05）：通过Logistic回归分析。饮酒过量、血清白蛋白降低、血清总胆红素升高是肝硬化腹水患者并发细菌性腹膜炎的危险因素，差异均有统计学意义（P〈0．05）。结论分析肝硬化腹水并发自发性细菌性腹膜炎的危险因素，可以为其临床诊断和治疗提供可靠的理论依据。Objective To approach risk factors of liver cirrhosis ascites complicated with spontaneous bacterial peri- tonitis. Methods 50 cases of liver cirrhosis ascites complicated with spontaneous bacterial peritonitis patients in Cen- tral Hospital of Hanzhong City from January 2014 to December 2015 were selected as observation group, then 50 cases of liver cirrhosis ascites withno spontaneous bacterial peritonitis patients were selected as control group. The single fac- tor conditions of sex, age, ALB, TBiL, blood platelet, prothrombin time international standardization ratio, duration of ascites, complications, cirrhosis formation reasons of the two groups were detected, the single factor conditions with statistical significance were analyzed by Logistic regression. Results Through single factor analysis, there were statistically signif- icant difference of the drink, ALB, TBiL between the two groups （P 〈 0.05）. Through Logistic regression analysis, over- drink, ALB reduction, TBiL rise were risk factors of liver cirrhosis ascites complicated with spontaneous bacterial peritonitis, the differences were statistically significant （P 〈 0.05）. Conclusion Analysis of risk factors of liver cirrhosis ascites com- plicated with spontaneous bacterial peritonitis can provide reliable theory basis for its clinical diagnosis and treatment.
