摘 要:目的探讨专职护士在食管癌术后患者肠内营养实施管理中的作用。方法选取2013年3月-2014年12月第四军医大学西京消化病医院收治的胸腹腔镜联合食管癌根治术后实施肠内营养治疗的198例患者.采用随机数字表法分为对照组和观察组,各99例。对照组采用责任制护理小组成员对患者进行肠内营养护理.观察组由专职护士负责肠内营养输注管理,比较两组患者肠内营养并发症发生率、健康教育效果及服务满意度、术后一般情况及术后管道并发症情况。结果观察组肠内营养并发症发生率显著低于对照组(P〈0.05)。观察组健康教育及服务满意优秀率显著高于对照组,差异有统计学意义(P〈0.05)。观察组首次排气及排便时间、住院时间均短于对照组,医疗费用低于对照组,差异有统计学意义(P〈0.05)。观察组术后非计划拔管率显著低于对照组(P〈0.05)。两组术后营养管堵管发生率比较,差异无统计学意义(P〉0.05)。结论食管癌术后实施肠内营养设置专职护士,可提高肠内营养护理质量,提高服务满意度,促进专科护理发展。Objective To explore effect of full-time nurse in the postoperative management of enteral nutrition for pa- tients with esophageal carcinoma. Methods 198 patients with implementation of enteral nutrition therapy after laparo- scopic combined thoracic esophageal cancer radical surgery from March 2013 to December 2014 were selected from Xijing Hospital of Digestive Diseases, the Fourth military Medical University. The patients were divided into control group and observation group by using the random number table method, each group had 99 cases. Primary nurse for postoperative management of enteral nutrition was used in control group, full-time nurse for management of enteral nu- trition was applied in observation group.The incidence rate of enteral nutrition complication, the health education effect, the satisfaction rate of service, the postoperative data and the pipeline related complication were compared between two groups. Results The incidence rate of enteral nutrition complication in observation group was lower than that in control group (P 〈 0.05). Excellence rate of health education and service satisfaction in observation group were higher than those in control group, with statistical differences (P 〈 0.05). The first time of exhaust and defecation time, length of hospital stay in observation group were shorter than those in control group, hospitalization cost in observation group was lower than that in control group, with statistical differences (P 〈 0.05). Postoperative unplanned extubation rate in ob- servation group was lower than that in control group, with statistical difference (P 〈 0.05). The incidence rate of post- operative nutritional tube wall tube were compared between two groups, with no statistical difference (P 〉 0.05). Con- clusion For esophageal carcinoma patients receiving enteral nutrition, full-time nurse setting can improve the nursing quality of enteral nutrition, increase service satisfaction rate, and promote the development of specific nursing.
