摘 要:目的:应用瞬时波强检测寸口弦脉参数,构建基于WI技术的弦脉脉象图,建立一种新型的脉诊技术。方法:应用ALOKA Prosoundα10彩色多普勒超声仪检测寸口弦脉与平和脉的WI各30例,从压力波(P)、血流波(U)、压力变化(dP·dt^(-1))与速度变化(dU·dt^(-1))、时间三维构建WI图像。结果:寸口平和脉波型特点:W1波幅较人迎脉为小,NA较人迎脉明显,W2甚至图形不明显或消失,R-1^(st)时间较人迎脉长;P波的重搏前波(潮波)出现较人迎脉为早,更靠近主波;人迎脉U主波向上呈"Λ"型,而寸口脉呈拱型圆钝"∩"型,且波幅为小。寸口弦脉波型特点:寸口弦脉U主波方向向上呈"Λ"型,而寸口平和脉呈"∩"型,且弦脉血流振幅较平和脉明显增大,呈徒增徒降趋势;NA波较深,W2波不明显;WI主波W1特征图型:弯月刀型。弦脉组W1、NA、W2、PWVβ较平和脉组增高,而弦脉组R-1^(st)、1^(st)-2^(nd)、AI较平和脉组减低,差异均有统计学意义(P<0.05)。结论:应用WI技术于脉象研究可使弦脉图更具可观性、可辨性、客观性,利于传统脉学的传承和发展。This study aimed to establish a new tool for traditional Chinese medicine(TCM) pulse diagnosis with constructing preliminary taut pulse images through testing taut cunkou pulse parameters via wave intensity(WI)technology.Thirty individuals with cunkou taut pulse and thirty individuals with normal pulse were included in this study.WI of cunkou normal pulse and taut pulse were examined by ALOKA Prosound a 10.WI and pulse images of the two types of pulses were constructed by three—dimensional levels of pressure wave(P),blood-flowvelocity wave(U),the product of blood pressure changes(dP·dt^(-1)) and blood-flow-velocity changes(dU·dt^(-1)),and time.As a result,compared with renying pulse,wave shape characteristics of cunkou normal pulse was as follows:W1 amplitude of normal pulse was smaller;NA wave was more obvious;while W2 wave was not obvious or disappeared;and the R—1^(st) time was longer.Furthermore,dicrotic wavefront pressure wave which also called tide wave appeared earlier than renying pulse,and was closer to the main wave.The U wave of renying pulse was " A " type,while cunkou pulse was arched blunt "fl" type,and the U wave amplitude of cunkou pulse was small.Wave shape characteristics of U wave in cunkou taut pulse was "A" type,while presented "fl" type in cunkou normal pulse.The blood flow amplitude of cunkou taut pulse was larger than that of cunkou normal pulse with a sharp rise and fall trend.The NA wave of cunkou taut pulse was deeper than that of cunkou normal pulse group,however,the W2 wave of cunkou taut pulse was not obvious.The graphic feature of W1 wave of cunkou taut pulse was like a crescent knife.Compared with cunkou normal pulse,W1,NA,W2 and PWVβ were increased,and the R-Ist,Ist-2nd and AI were decreased with statistical differences.It was concluded that the application of WI technology in pulse study made taut pulse figures more visible,recognizable and objective,which was conducive to inherit,spread and develop Chinese traditional
