干扰素治疗HCV合并HIV感染患者的眼底观察

摘  要:目的观察使用干扰素治疗丙型肝炎病毒(HCV)合并人类免疫缺陷病毒(HIV)感染患者的眼底特征变化。方法对广州市第八人民医院2009年1月至2014年12月之间收治的69例HCV合并HIV患者,进行全身检查和专科眼部检查。69例患者全部应用聚乙二醇干扰素(Peg-INF)皮下注射治疗,180μg/次,1次/周。结果 HCV合并HIV感染患者皮下注射Peg-INF-α-2a 24周以上后,CD4+T淋巴细胞计数为(328±98)个/μl,显著低于使用干扰素前(410±102)个/μl,差异有统计学意义(P<0.05)。血细胞计数主要表现为白细胞(WBC)和血小板(Plt)计数下降(P<0.05)。眼底出现棉绒斑11例20只眼(占患病例数15.9%),线状出血7例10只眼(占患病例数10.1%),火焰状出血3例5只眼(占患病例数4.3%),棉绒斑合并出血4例(占患病例数5.8%)。结论干扰素可以引起HIV合并HCV患者眼底改变,临床上要定期复查。Objective To observe the fundus characteristics of HIV and HCV coinfected patients with the treatment of interferon. Methods Data of patients(n =69) was coming from Guangzhou NO.8 People′ s Hospital between January of2009 and December of 2014. All patients were made systematic and professional ocular examinations, and treated with subcutaneous injection of Pegylated Interferon IFN-2a at a dose of 180 g, once a week. Results Patients were treated with 180 g IFN-2a via subcutaneous injection for 24 weeks. After treatment,CD4+T cell counts(328 ±98 / μl)was dramatically lower than that of before injection(410 ±102 / μl)(P〈0.05);WBC and platelet counts were significantly decreased. Fundus characteristics of the patients showed 11 cases of cotton wool spots(15.9%),7 cases of linear hemorrhage(10.1%),3 cases of flame-shaped hemorrhage(4.3%),and 4 cases of cotton wool spots plus bleeding(5.8%). Conclusion The treatment of HIV and HCV coinfected patients with interferon is safe and effective in short period, but showing changes in ocular fundus. Therefore, regular reexamination plays critical role in clinical therapy.
